&NA;The effects of isoxsuprine on the myometrium were examinedin vitroin 12 uterine strips of 6 rabbits andin situin 12 rats and 20 dysmenorrheic patients.In vitro,isoxsuprine reduced isometric tension in both electrically stimulated and spontaneously contracting uteri. At 30 minutes after treatment, 200 μg/ml isoxsuprine suppressed tension by 60% (P< 0.001). In unanesthetized rats, 500 μg isoxsuprine reduced the intrauterine pressure (IUP) by 37% (P< 0.001), and 1000 μg by 45% (P< 0.001), during 76 ± 14 and 100 ± 4 minutes, respectively, The frequency of IUP cycles was also reduced significantly (P< 0.001). In menstruating patients, 60 mg isoxsuprine reduced the IUP during 2 hours by 46% (P< 0.001) and decreased the subjective symptoms of pain (P< 0.001). The frequency of IUP cycles was also reduced significantly (P< 0.001). The suppression of IUP was explained by the reduction of wall tension (WT), since IUP = 2WT/R (where WT = uterine wall tension and R = radius). This combination ofin vitroandin situstudies in “animal models” and patients is recommended for the quantitation of the therapeutic actions of drugs.

&NA;Study was made of the potential clinical utility of a random urine estrogen/creatine (E/C) ratio as an index of fetal welfare. Detailed analysis of estriol, total estrogen, and creatinine measurements as determined on biweekly 24‐hour and paired random urine samples in 20 pregnant subjects during the last 20 weeks of pregnancy was carried out. Results revealed that total urinary estrogen excretion bore a relatively constant relationship to estriol excretion, that 24‐hour creatinine excretion remained quite constant, and that values of E/C ratio in the 24‐hour urine and the paired random urine demonstrated excellent correlation. It was possible to predict the 24‐hour urine E/C ratio from the paired random urine E/C ratio to within less than ±5 with 95% confidence. A proposed practical approach to the use of the random urine E/C ratio as an index of fetal welfare is presented.

&NA;A recorder is described which is capable of continuously measuring intervals between two events of the fetal cardiac cycle, as well as functioning as a routine intrapartum monitor. We have primarily measured the interval between the R wave of the fetal ECG and the second fetal heart sound (R‐S2), but the recorder is also capable of measuring R‐R, R‐A0(opening of aortic valve), R‐PI (placenta), and R‐Sc (scalp) intervals. In the discussion, we endeavor to show that these intervals reflect fetal cardiac performance, which is altered by drugs and acidemia.

&NA;In order to evaluate the most reliable means of determining fetal maturity, six amniotic fluid components were analyzed from 99 women who were 22 to 40 weeks pregnant. The six parameters investigated were: amylase activity, the lecithin to sphingomyelin (L/S) ratio, percentage of fat cells, and the concentrations of protein, creatinine, and bilirubin. In general, single components and combinations of two or three of the tests did not significantly enhance the predictive accuracy or reduce the error from that obtained with the L/S ratio used alone.

&NA;Cytologic diagnosis of adenocarcinoma metastatic to the peritoneal surface is often hampered by the difficulty of differentiating between reactive mesothelial cells and macrophages from malignant epithelial cells by light microscopy. Within vitrotissue culture technics, the fibroblastic dedifferentiation of mesothelial cells facilitates the recognition of residual neoplastic epithelial cells. Ascitic fluid from 18 patients who ultimately had histologic documentation of intraabdominal adenocarcinomatosis was studied by tissue culture technics. In 15 of the 18 patients, small islets of replicating tumor cells could be identified after the mesothelial cells had dedifferentiated into fibroblasts. With one exception the average time between collection of the specimen and recognition of adenocarcinoma was 6 days. In 14 cases, concomitant cytologic analysis was available. In 10 cases, a definitive diagnosis of malignancy was made by cytologic analysis.

&NA;An evaluation and a critique of prognostic parameters and criteria involved in the classification and clinical staging of epidermoid carcinoma of the vulva are presented, comparing the FIGO system and a proposed more definitive system. Biostatistical as well as clinical evidence for an improved system is presented. The proposed system is based on statistically valid data, has good patient distribution, and an orderly progression for therapy and prognosis. In addition, it is well suited for computer programming and for the comparison of patients and therapy, as well as for delineating the natural history of vulvar cancer.

&NA;Needle aspiration of a breast mass, a procedure widely employed in Scandinavia but neglected in the United States, has an important role in the diagnosis of carcinoma of the breast. Five hundred and ninety‐seven patients had aspirations from 706 lesions, 415 solid and 291 cystic. There were 83 malignancies, with abnormal cells in 73. In the majority the diagnosis could be made readily. There is no known contraindication to the procedure. Aspiration at the initial visit may alert the physician to malignancy, thus avoiding undue delay in treatment.

&NA;The authors point out the difficulty in recognizing puerperal myocardiopathy and the necessity for standardizing the diagnostic features. The patient described here was all the more unusual because of the coincidental occurrence of a second rare condition—simultaneous uterine and tubal pregnancies. Our data may add support to the theory that an ill‐defined myocardial lesion results from an autoimmune response.

&NA;Hourly fetal urine production rate (HFUPR) was measured in a multiparous woman with diabetes mellitus and an excessive polyhydramnios. HFUPR values appeared to be in the normal range for gestational age. In the case presented, the origin of the polyhydramnios could not be attributed to excess production of fetal urine but should be sought in an imbalance of fetal‐maternal water exchange on the basis of a still unknown pathophysiologic mechanism.
