Forty-five pregnancies in patients with homozygous hemoglobin S disease and 9 pregnancies in patients with sickle cell-hemoglobin C disease are reported. An increased incidence of toxemic and infectious complications was noted. Severe anemia was seen quite often, and many patients developed some degree of cardiac decompensation. Bone pain crises were frequent. Many patients went into labor prematurely, and infants small for gestational age often were delivered. Perinatal mortality rates were 24% in homozygous hemoglobin S disease and 62% in sickle cell-hemoglobin C disease. Four maternal deaths attributable to these hemoglobinopathies are reported. Increased use of surgical sterilization and therapeutic abortion in these patients seems to be justified.

Pregnancy is possible but hazardous for women with cystic fibrosis (CF). The obstetric care of the patient presented was complicated by pneumonia, premature labor, hypotension and abruptio placentae. Therapeutic abortion is advised to preserve maternal health in selected cases. Contraceptive counseling is urged for girls with CF who reach menarche.

Pregnancy in a patient with a well-established diagnosis of aplastic anemia is described. Successful delivery by cesarean section and splenectomy, after large amounts of platelet concentrates were administered preoperatively and postoperatively, played an important role in the management of this severely pancytopenic patient.

Investigations using PGF2a, for therapeutic abortion are currently in progress in several centers. This case report describes an asthmatic attack during intravenous administration of PGF2a, to a patient with a history of bronchial asthma. Moderately severe bronchoconstriction was noted clinically during infusion of 200 μg/min. The symptoms cleared rapidly soon after the drug was discontinued. The literature on respiratory effects of prostaglandins is reviewed.

The second surviving patient who sustained spontaneous rupture of a renal artery aneurysm during pregnancy is presented. Awareness that these aneurysms rupture more commonly during pregnancy will result in early diagnosis and adequate therapy, hopefully reducing maternal mortality. To find these aneurysms before they rupture, arteriography should be used more frequently in the workup of patients who develop hypertension or who have abdominal bruits. Nephrectomy is indicated in the presence of massive renal or retroperitoneal hemorrhage during pregnancy, even when the source of bleeding cannot be definitely ascertained.

Prolonged postoral contraceptive amenorrhea in 41 women did not correlate with physical status, other endocrine abnormality or duration of pill therapy but did correlate with past menstrual dysfunction and/or absence of withdrawal bleeding during pill use. Duration of oral contraception did not correlate with length of amenorrhea or therapeutic response. Six patients had galactorrhea; while this probably does not alter prognosis, patients should be observed for a pituitary lesion. Menses resumed in 69% of the untreated women, 69% of the women treated with clomiphene and 42% after other agents. Patients with low serum LH and low total urinary estrogen levels responded poorly to clomiphene. A crude incidence of postpill amenorrhea of 2.2 per 1000 was noted among almost 20,000 oral contraceptive users.

Inhibands were inserted into 236 parous women who were followed for a 2-year period. During the study, 46 women underwent elective hysterectomy and 41 patients were lost to followup; thus, the data are not suitable for the preferred life table method of analysis. Pregnancy occurred in 25 patients, expulsion in 14 and the device had to be removed for medical reasons in 53. One perforation occurred 6 months after insertion, and in 11 of 24 specimens examined at hysterectomy, the device was embedded, usually in the lower uterine segment. Side effects, such as bleeding and cramping, were reported by over 50% of the patients during the study. The incidence of serious problems, such as pregnancy, embedding and inability to remove the device, probably due to embedding, increased with time.

A surveillance system was organized to monitor the impact of the liberalized Maryland Abortion Law of July 1968. During the first year, 2134 hospital abortions were reported; 5530 were reported in the second year and 3210 during the first 6 months of the third year. The predominant indication for abortion has been mental health (91 to 96%), and the most frequently used method is now suction curettage. Women obtaining abortions are almost entirely Maryland residents (97%), young (median age 22), of low parity (50% with no living children) and most often unmarried (68%). Complication rates have been highest for procedures performed late in gestation, and 3 deaths have been associated with procedures after 16 weeks' gestation.
