Seven patients With massive condylomata complicating pregnancy were treated with electrocoagulation and curettage. Six were operated upon in the mid-second trimester and 1 in the early third trimester of pregnancy. Results were surgically satisfactory in all patients. Six have since delivered vaginally. In only 1 case was blood loss excessive, and this patient had the most extensive disease. Electrocoagulation and curettage is recommended as definitive therapy of massive condylomata acuminata during pregnancy.

FulminatingStreptococcus pyogenespelvic inflammatory disease and peritonitis occurred in a previously healthy 29-year-old woman 4 months after a Dalkon Shield intrauterine device was inserted. Despite intensive treatment, the patient died of septic shock 63 hours after the onset of her illness. The clinical course and significance of severeStreptococcus pyogenesinfections arc discussed. Other deaths from pelvic sepsis associated with intrauterine devices are reviewed.

The Mersilene mesh, single-layered gauze-hammock has been used successfully as a modification of the Aldridge sling procedure for certain cases of severe or recurrent urinary stress incontinence. The technic is described and, in a series of 22 cases treated by this procedure since 1969, there has been no instance of sinus formation, foreign body rejection or urethral necrosis.

A giant cervical polyp was discovered in a nulliparous woman, -1 years postmenopausal. Symptoms included leukorrhea and periodic vaginal bleeding of 2 years duration. To our knowledge this is the first report in the literature of a benign cervical lesion of this size. Management, treatment and pathologic findings of this extremely large, cervical lesion are discussed.

The correlation between cytologic response to estrogen therapy and clinical symptomatic improvement, endometrial histology and withdrawal bleeding from a progestational compound was studied in postmenopausal women. Two clinical investigations were carried out: one in a group of surgical menopausal subjects who were treated with 1.25 mg of conjugated estrogens each day for 21 days; after which the medication regimen was interrupted for 7 days. In the second group, natural postmenopausal patients were given a continuous low dose, 0.625 mg of conjugated estrogens, supplemented by a progestational agent approximately every 3 months. The results indicate that each individual responded differently to the same total amount of estrogen administered. However, a similar degree of cytologic stimulation was observed in each subject at each treatment cycle. A change in symptoms did not necessarily correspond to the degree to which the cytogram changed, but the endometrial response and the success or failure of inducing withdrawal bleeding by a progestational compound correlated well with the total superficial cell count.

Cervical cerclage procedures were performed on 25 patients at the UCLA Hospital from 1958 to 1971. A total of 38 operations of four types were performed on these patients during a time interval corresponding to 26,474 deliveries. These 38 operations were followed by 29 live births and 9 failures. The cumulated preoperative fetal salvage was 26% of 120 pregnancies, which includes 51 mid-trimester losses. The postoperative salvage rate, including surviving premature deliveries, was 76%. A failure rate of 50% was noted in patients sutured after 20 weeks, compared to 11% in patients sutured before 20 weeks' gestation. Significant complications were found in 18 cases, including infections, cervical stenosis and subsequent operative delivery.

Electrocautery was the only therapy in 148 patients with cervical intraepithelial neoplasia diagnosed by colposcopically guided biopsy. The disease was completely eradicated, as demonstrated by subsequent cytologic and/or histo-Iogic studies, in 92% of the patients and no invasive cancer was found. Excellent correlation between abnormal cytology and persistent intraepithelial neoplasia was demonstrated during followup. The criteria for selecting electrocauterization in the outpatient treatment of cervical intraepithelial neoplasia were discussed.

The 189 patients with clinical Stage I carcinoma of the uterine cervix treated during the 10-year period, September 1, 1961 to September 1, 1971 are reviewed. One hundred fifty-six of the patients, or 83% of the total series, were selected for primary surgical treatment, 150 by radical Wertheim-Taussig hysterectomy. Thirty-two patients received primary radiation. Operative and long-term complications were extremely low, with no urinary or bowel fistulae. The 5-year actuarial survival rate of the operated patients is 93%. The corrected 5-year actuarial survival rate of the 32 radiated patients is 88%.

Choriocarcinoma is reported in a patient in whom gonadoblastoma had been diagnosed previously. Based on Scully's review, this case report places the coincidence of gonadoblastoma and choriocarcinoma at I in 75, and the coincidence of aggressive germ cell tumors and gonadoblastoma at 10% or more. This experience supports the contention that when gonadoblastoma is found in one gonad, the opposite gonad should be removed.
