That the sexually active, lower income teenage female has a significant risk of developing cervical carcinoma in situ was the conclusion we reached after reviewing the statistics of a cervical cancer screening project covering a 5-year period of time. A total of 7520 teenage females had been screened as of January 1, 1972. This represented about 33% of the total screening effort. Of these, a total of 51 patients were found, on cytologic smear, to have cells sufficiently abnormal to warrant tissue diagnosis. There were 12 confirmed cases of carcinoma in situ, and 32 cases showed varying degrees of cervical dysplasia. Thus, carcinoma in situ occurred at a rate of I per 655 teenagers screened. The authors feel that with this rate, it is certainly worth the time, effort and cost required to include any sexually active female in all cervical cancer screening projects, regardless of her age.

An assessment was made of the prophylactic effect of Endoxan and Bleomycin on 20-methylcholanthrene-induced cervical carcinoma in mice receiving the drugs from the early stage of experimental carcinogenesis. Marked inhibition of carcinomatous development and salient suppression of atypical epithelial proliferation were evident in animals treated with the anticancer chemotherapeutic agents. Administration of the drugs in early precancerous stage was observed to be more effective.

The records of 432 patients with breast cancer who underwent surgical castration during the years 1955 through 1967 were reviewed to determine whether subsequent pelvic difficulty justified hysterectomy. Cases of oophorectomy alone were compared with those of oophorectomy and hysterectomy. The following factors were considered: age, operative complications, incidence of ovarian metastases, uterine pathology, blood transfusions, duration of anesthesia, postoperative vaginal bleeding, and morbidity and mortality rates. Morbidity was slightly higher and hospitalization slightly prolonged in patients undergoing hysterectomy. As to uterine pathology, a high proportion of the uteri were abnormal and some had malignant disease. The likely incidence of subsequent pelvic problems after oophorectomy alone was assessed as 1 in 25. Hysterectomy is probably indicated in all patients with breast cancer undergoing castration, except in those with a poor operative risk due to extensive metastatic disease.

With the advent or the flexible uterine curette, outpatient suction curettage is becoming the preferred method of elective abortion for pregnancy of 10 weeks' gestation and under. The present study reports on 1 year's experience using a simple water suction pump in conjunction with the 6 mm flexible uterine curette. The effectiveness of the water vacuum suction curettage (WVSC) unit is compared with that of the electric suction pump. Four-hundred patients who subsequently underwent WVSC experienced a postoperative morbidity rate of 3.0%.

An unusual case of tubo-ovarian abscess due toActinomyces bovisassociated with a metailic intrautcrine contraceptive device is reported. One other case of pelvic actinomycosis and 8 cases of actinomycosis of other sites are discussed and the literature reviewed. The pathogenesis of this disease is unclear but the treatment consists of large doses of penicillin and surgical excision.

One hundred fifty-two examples of chronic endometritis were reviewed to demonstrate the clinical significance in two population groups— one from this country, the other, Colombia, South America. The histologic diagnosis was based solely upon the presence of plasma cells in the endometrial stroma. No correlation was found between the number of plasma cells and the severity of symptomatology. The most common symptoms were menstrual disturbances, present in virtually every patient, and pelvic pain or tenderness, found in 50% of the women. Chronic endometritis could be attributed to a specific etiologic factor in 84% of the patients: pelvic inflammatory disease in 25%, intrauterine contraceptive devices in 14%, postpartum factors in 12% and postabortal factors in 41%. These findings indicate that chronic endometritis is a real clinicopathologic entity with a demonstrable cause in most instances. It should be considered in the differential diagnosis of all women with irregular menses and pelvic pain.

The Center for Reproductive and Sexual Health is an ambulatory abortion clinic performing first trimester procedures under paracervical block. Sixty thousand (60,000) procedures have been performed from July 1, 1970, to August 1, 1972. A syndrome of syncope, abdominal pain, and tenderness several hours postprocedure, closely resembling the picture of uterine perforation, has been identified and is called, for want of a better understanding of the pathophysiology involved, the postabortal pain or “redo” syndrome. It is promptly relieved by resuctioning (“redoing”) the patient. Because of the masses of firm clot found in the uterus at the time of resuctioning, it is suggested that a significant coagulopathy resembling that previously described in second trimester abortion may be operative here.

Human cervical mucus lipid contains 10.5% free fatty acids in an entire pooled cycle. Cyclic variations of the amount of free fatty acid are observed in the menstrual cycle. It is noted that the higher levels of free fatty acids are in the estrogenic phase, compared to the progestational phase of the cycle. The amount of saturated fatty acids varies from 75 to 84% in different phases of the cycle. The most abundant fatty acids are myristic, palmitic, stearic, oleic and linoleic. Oral contraceptives alter the free fatty acid composition and the fatty acids tend to become saturated, but the amount of change is small. The major fatty acids in mucus of both users and nonusers of oral contraceptives are palmitic and stearic.

The results of a prospective study of the effectiveness and safety of saline instillation with oxytocin administration in midtrimester abortion is presented. One instillation (14 weeks) failed to result in abortion. The mean time from instillation to delivery was 23.4 hours, with a significantly shorter interval in multiparas. Retained placentas were noted in 29% of cases. A curettage was required in 4.6% of cases. The infection rate was less than 1%. Water intoxication with convulsion was questioned in 1 case. The combination of saline instillation with oxytocin administration is considered an effective and safe procedure when precautionary measures outlined are adhered
