&NA;Studies to determine the total number of circulating lymphocytes and their morphologic transformation in the presence of phytohemagglutinin and pokeweed mitogen were carried out in 156 women: 81 who currently had an ovarian cancer or who had had it in the past; 54 normal women; and 21 patients with malignant diseases. The latter two groups served as controls. The total number of circulating lymphocytes is an important consideration, as there is a significant decrease of lymphocytes in patients with ovarian cancer, a factor with great bearing on prognosis. The lymphoblastic transformation test in the presence of phytohemagglutinin and pokeweed mitogen showed not only significant changes in patients with ovarian cancer but also a correlation with survival at 6 months.

&NA;Female sterilization by laparoscopy has become widely accepted throughout the United States and Europe. The failure rate of this procedure is still speculative. In the course of 4200 operative sterilizations, 32 failures have been identified and 31 of these have been subsequently evaluated. The most common finding was inadequate or superficial fulguration and resection. Operative errors such as round ligament fulguration were also more common than expected.

&NA;Records of 108 patients undergoing combined vacuum aspiration and laparoscopic sterilization in an outpatient surgical program were reviewed and compared with those of 195 patients who underwent abortion only in the same outpatient program. Mean operating time for the combined procedure was 30 minutes; total mean hospitalization time was 5 hours and 7 minutes. Complication rates for the combined procedure and for abortion alone were 9.2 and 7.2%, respectively. Subsequent hospitalization was necessary for 4.7% of patients undergoing the combined procedure and 3.1% of those having abortion only. Laparoscopic sterilization has been found to add no significant morbidity but has markedly reduced cost and hospitalization for the patient desiring permanent contraception following first trimester abortion.

&NA;In reviewing maternal mortality in Minnesota over the last 10‐year period, obesity proved to be a major high‐risk factor to the parturient. In the following report 12% of the study group weighed over 200 1b in the nonpregnant state. Pulmonary embolism was the leading cause of death in the obese group. Obstetricians are encouraged to recognize the possible complications that obesity poses in the pregnancy period.

&NA;A study of 20 patients with hemoglobin E trait abnormality and 8 patients with hemoglobin E disease in pregnancy is presented. The hematologic abnormality in HbE hemoglobinopathy is mild though hemoglobin values are significantly lower than in HbA patients. Fetal wastage is not increased and the salvage rate is high (live births over 95%). The mean birthweight of the baby in HbE patients is significantly lower than in HbA patients, but the incidence of prematurity is not significantly increased in HbE patients. Blood loss at delivery is not markedly affected by the hemoglobin type. The incidences of postpartum hemorrhage, preeclampsia, and malaria are not significantly increased in HbE patients; the reason for the significant increase in incidence of abruptio placentae in HbE is not known. No maternal deaths or congenital fetal abnormalities were recorded. It is concluded that the course of pregnancy in HbE trait abnormality and HbE disease is relatively benign.

&NA;Report on a case of sclerosing stromal tumor of the ovary with detailed pre‐ and postoperative hormonal analysis and an electron microscopic study of the tumor. The tumor was secreting both estrogenic and androgenic hormones. Ultrastructural studies have corroborated previous light microscopic findings on the variegated appearance of the tumor and the abundance of collagen among the cells and indicated that the tumor is functionally active and of ovarian stromal origin.

&NA;A 68‐year‐old postmenopausal patient with adenoid cystic carcinoma of the cervix is described. Thirty‐two other cases were collected from the literature, and a clinicopathologic analysis for the whole group is presented. The study indicates thata) adenoid cystic carcinoma usually occurs in postmenopausal patients in the seventh decade;b) spotting and bleeding are the commonest presenting problems;c) vaginal cytology was abnormal in most cases in which it was reported;d) the lesion usually forms a mass and infiltrates parametrial and paracervical tissues;e) the tumor is frequently associated with squamous cell carcinoma (in situor invasive) or other types of cancer;f) local recurrence after radical surgery and/or irradiation, as well as pelvic and widespread metastases, can occur; andg) the histogenesis of this type of tumor is unclear. The various possibilities are discussed.

&NA;Plasma steroids, estrogen, progesterone, and testosterone, and plasma gonadotropins FSH and LH were determined before and after adrenal suppression in an 18‐year‐old patient with gonadal dysgenesis (XX/XO). Total plasma estrogen was initially low (9.2 ± 2.4 pg/ml) and did not change after adrenal suppression. Plasma progesterone (380 ± 18 pg/ml) and testosterone (236 ± 8.6 pg/ml) were in the range found in the normal follicular phase and fell to 50 ± 17 pg/ml and 112 ± 2.0 pg/ml, respectively, after adrenal suppression. Mean levels of FSH and LH were unchanged after adrenal suppression. These results suggest that the adrenal may make a significant contribution to plasma levels of progesterone and testosterone in patients with gonadal dysgenesis and may also contribute to the plasma levels of these steroids during the follicular phase of the menstrual cycle.

&NA;Aneurysms rarely present as a complication of hysterectomy. A case is reported of a left common iliac artery aneurysm causing readmission 2 weeks after total abdominal hysterectomy. The aneurysm remained undiagnosed prior to percutaneous rupture and subsequent laparotomy. The aneurysm was ligated and resected. No graft or arterial repair was attempted because of the infection that was present. Twenty‐three days after hysterectomy, the patient underwent above‐the‐knee amputation. A review of the literature is undertaken and possible etiologic mechanisms discussed.
