Fifty-eight gravidas who fell in the high-risk pregnancy category were followed by both single and serial metabolic urinary estriol determinations during their last trimester. Forty-one women (Group A) gave birth to viable babies; 17 (Group B) had babies who died. The concept of decreased urinary estriol output as an indicator of fetoplacental dysfunction has been confirmed, since total estriol levels were almost uniformly lower during the entire trimester in the group of women whose babies died, than in the group who gave birth to viable babies. A new concept of considering estriol concentration (milligrams per liter of urine per 24 hr) has been introduced and may be of value where total estriol output is in a difficult-to-interpret range—“ the shadow zone.”

The oral contraceptive, Ovulen* (ethynodiol diacetate and mestranol) has been found to increase plasma triglyceride by 76% in a group of premenopausal women. The elevation in triglycerides was associated with increases in pre-β-lipoprotein. Within this group, 7 women with cholesterol levels averaging 160 mg/100 ml showed a 44% rise in cholesterol, and then continued at the same level as the 12 women whose cholesterol averaged 207 mg/100 ml without change during therapy. Phospholipids rose significantly to 19% above control values. Mechanisms for the lipid elevations are discussed.

Whether or not the endocervical columnar epithelium has a histologic cycle is still doubtful. Histochemical stains did reveal an acid mucosubstance which concentrated in the apical portions of the epithelial cells during menses and which was more homogeneous throughout the cell during the remainder of the cycle. Sequential contraceptive therapy did not alter this finding but did significantly increase the amount of acid mucosubstance present in the glandular cells and in the epithelium lining the endocervical canal. The longer the sequential agent was used, the greater the increase in mucosubstances.

A group of 1020 women attending a planned parenthood clinic were screened cytologically by the cervical-scrape technic. Twenty-seven of these women diagnosed as having dysplasia or carcinoma in situ were suitable and available for prolonged research investigation. The age distribution of the 27 patients was 19–39 years, with 77.7% between 20 and 30. Nine were single; 6 were divorced. Of the 27 women, almost two-thirds (17) had persistence of cellular lesions, without alteration, following continuous heavy cyclic Enovid medication. One-third of the group so treated (9 patients) showed regression of lesions. No biopsy was performed or anticancer treatment administered during the period of investigation.

Traumatic rupture of the placenta resulting from an automobile accident during the thirtyfifth week of pregnancy is reported. The mechanism of placental rupture which caused fetal death by exsanguination into the amniotic sac, without harm to the mother or premature delivery, is discussed.

Four patients are reported with massive edema of the ovary which was mistaken grossly for neoplasm at celiotomy. The ovaries were large, white, and wet. Microscopically, they resembled soft fibromas and consisted of hyperplastic cortical tissue separated by proteinaceous fluid. In one instance, there was an associated luteincell hyperplasia; this patient had evidence of masculinization. The other 3 patients presented with abdominal pain. The most probable pathogenesis is partial torsion, possibly recurrent, of the mesovarium. Consideration should be given to the preservation of such ovaries by securing them in a position from which they cannot twist.

Laparoscopy was performed on 114 patients under general anesthesia and no major complications resulted. The authors cite the method's advantages over pelvic endoscopic technics. It was used to achieve permanent tubal sterilization in 44 patients.

The X-ray findings were reviewed in 17 of 30 patients who had had advanced extrauterine pregnancy observed between 1946 and 1967. The presence of the following radiologic signs, either separately or in combination, were found to be diagnostically significant: abnormal fetal presentation and position, especially transverse lie with the fetal spine cephalad; failure to visualize the uterine shadow around the fetus; intermingling of the mother's intestinal gaseous shadows with the fetal parts; and, practically pathognomonic, fetal parts posterior to the mother's lumbar spine (lateral film). A hysterogram also demonstrates the uterine cavity as separate from the fetus, and occasionally there is visualization of a uterine mass separate from the fetus on soft-tissue X-ray study.

The cellular composition of amniotic fluid from 53 gravidas was studied by cytologic filtration and Papanicolaou staining of aspirated samples obtained at various stages during the latter half of gestation. Epidermal cells were predominant and were classified into five categories based on morphologic characteristics: anucleated squamous, precornified, cornified, intermediate, and parabasal cells. Parabasal cells were rare after 36 weeks except following intrauterine death. The proportion of intermediate and anucleated squamous cells was markedly decreased after 36 weeks, when precornified became predominant and cornified cells increased. The latter change preceded the increase of Nile blue-stained orange cells. Additional observations were the rarity of sebaceous and amniotic membrane cells, autolytic and inflammatory changes following fetal death, and the presence of neutrophils in amniotic fluid during labor.

A study of fetal base determinations as an indicator of neonatal condition, using the Apgar score as a reference point, was carried out in the fetuses of 194 parturients. Correlation coefficients for fetal base deficit and Apgar score were not good, except for low 5-min Apgar scores. The generally poor reliability of the fetal base deficit values, compared to Apgar scores, may be attributed to the long time between fetal blood sampling and delivery as well as the FHR pattern present when the sample happened to be taken.
