Thirteen cases of documented ventriculomegaly were diagnosed between 19 and 24 weeks of gestation. In 11 cases, the lateral ventricle/hemispheric width ratio was abnormally increased at the time of the first ultrasound examination, and in two, the lateral ventricle/hemispheric width ratios were initially in the high normal range. Severe ventriculomegaly was observed in both of the latter cases when repeat examinations were performed two weeks later. In 12 cases (92%), the biparietal diameter (BPD) was appropriate for gestational age, whereas the BPD was increased in only one. Two of the affected fetuses were found in conjunction with normal twins. This series suggests that fetal ventriculomegaly can be accurately diagnosed before fetal viability using the lateral ventricle/hemispheric width ratio.

Circadian rhythms have been identified in a variety of maternal and fetal biophysical and endocrinologic parameters. The authors have undertaken a 24-hour study to identify the normal variation in fetal bladder volumes in the healthy, term human fetus. A significant decrease in fetal bladder volumes occurred between 2400 hours and 0600 hours when compared with other times of the day. It is suggested that this fall in fetal bladder volumes may be related to fetal cardiovascular or adrenal gland function.

Two to four stems ofLaminariawere inserted into the cervixes of seven healthy women at second trimester. Fortyeight hours later, theLaminariain each patient were replaced with a rubber balloon (100 mL capacity) that was inflated with 150 to 180 mL of physiologic saline solution, and traction of 800 to 1000 g was applied for an additional 24 hours. During Laminaria application, marked cervical softening was noted in all cases, and weak but definite uterine contractions were initiated in three women. During the balloon application regular uterine contractions were initiated or increased in all patients, with three late second trimester patients aborting within 24 hours. Light and electron microscopic studies of the decidua parietalis obtained during apparent uterine activity and after abortion revealed normal cell morphology. A significant increase in amniotic fluid F prostaglandin was noted after the balloon treatment (P< .05). The authors conclude that uterine stretching induces uterine activity and a rise of prostaglandin in amniotic fluid without disfunction of decidual cells.

Tampons were tested for effect on growth and production of toxic shock syndrome toxin 1 byStaphylococcus aureus. Under good growth conditions, regular absorbency tampons had little effect on bacterial growth and inhibited toxin production two- to four-fold. In contrast, higher absorbency tampons had three different effects: 1) some tampons had no effect on bacterial growth but inhibited toxin production; 2) many tampons inhibited both growth and toxin production; 3) one tampon inhibited growth but increased exotoxin per cell. These effects were independent of degree of saturation of the tampons and were observed at incubation times of six, 12, and 18 hours. In no instance was the production of toxic shock syndrome toxin 1 per milliliter increased in the presence of tampons when compared with control.

Laparoscopy was used to evaluate 100 women who consistently reported pelvic pain in the same location for a minimum of six months. These findings were compared with those of 50 asymptomatic women who underwent laparoscopy for tubal ligation. Overall, 83% of the group with pelvic pain had abnormal pelvic organs as compared with 29% of the asymptomatic group. Adhesions were the most common pathology accounting for 38% and pelvic endometriosis accounted for 32% of the symptomatic group. Results of this study suggest that pelvic pain reported to be in the same location for a minimum of six months, is usually associated with organic pathology.

Macroscopic and histologic findings in 11 patients operated on for isolated tubal torsion suggested a common pathophysiologic process. During laparotomy, tubal torsion appeared to involve the fimbrial end of the fallopian tube, distal to the site of compression by the sterilization procedure, the ovarian ligament, or a dense adhesion. Histologic study of the tube showed signs of vascular disturbances. Similar signs, although to a lesser extent, were present in the fimbrial end of the contralateral tube in sterilized patients. A causative mechanism resulting in tubal torsion is proposed. The present report suggests that tubal torsion can be a late complication caused by some methods of tubal sterilization.

Among 48 women with long-term follow-up after microsurgical reversal of tubal sterilization, the pregnancy rate correlated closely to the length of tube on the longest side. Aside from an accurate anastomosis, no other factor significantly affected pregnancy rate.

Hormonal data were evaluated in 34 women with acne resistant to usual forms of therapy. Radioimmunoassay (RIA) was used to measure serum testosterone, dehydroepiandrosterone sulfate, 17 α-hydroxy-progesterone, prolactin, progesterone, free-testosterone, and testosterone index. Eighty-eight percent of these women were found to have androgen excess. Using the Dexamethasone Suppresion Test, the sources of the hormonal abnormalities were identified and found to be adrenal in 24%, ovarian in 38%, and mixed adrenal and ovarian in 26% of the patients. The data supports the contention that women with acne that persists despite the common therapeutic modalities, warrant endocrinologic evaluation.

The clinical and laboratory effects of low-dose heparin prophylaxis was prospectively studied in a controlled trial of 182 patients undergoing major surgery for gynecologic malignancy. Low-dose heparin was given in 5000 U subcutaneously two hours preoperatively and every 12 hours for seven days postoperatively. Low-dose heparin-treated patients had a significantly increased daily retroperitoneal hemovac drainage. Although not statistically significant, low-dose heparin was associated with increased estimated intraoperative blood loss, transfusion requirements, and wound hematomas. Fifteen percent of patients receiving low-dose heparin were found to have an activated partial thromboplastin time greater than 1.5 times the control value. In these patients, all clinical bleeding parameters were significantly increased. Low-dose heparin-treated patients also had significantly prolonged activated partial thromboplastin time and lower final platelet counts as compared with the control patients. When using low-dose heparin for thromboembolism prophylaxis, patients should be closely observed for clinical hemorrhagic complications. Activated partial thromboplastin times and platelet counts should be monitored throughout therapy.

Dermatomyositis-polymyositis was associated with gynecologic carcinoma in ten patients treated at the Mayo Clinic between 1952 and 1982. Only two patients did not manifest the typical skin changes of acute dermatomyositis. In all ten cases, the onset of dermatomyositis–polymyositis preceded evidence of carcinoma. The intervals between initial symptoms of dermatomyositis–polymyositis and subsequent diagnosis of carcinoma ranged from three months to six years. Usually, the carcinoma became evident within two years. The carcinomas were ovarian in five cases, cervical in three, endometrial in one, and vaginal in one. Ages of the ten patients ranged from 40 to 66 years (mean, 53 years). In most cases, symptoms of dermatomyositis-polymyositis regressed markedly after diagnosis and treatment of the underlying carcinoma. In selected patients for whom more detailed follow-up information was available, the manifestations of dermatomyositis-polymyositis progressed and regressed in synchrony with tumor spread and effective treatment, respectively.
