A case report of spontaneous rupture of a septate uterus at 18 weeks' gestation due to placenta percreta is presented. Similar cases reported in the literature are reviewed. The etiology and methods of treatment are discussed.

A monozygous set of twins is reported. One member of the twins is malformed while the other is normal. The etiology of the discordance is discussed and cytogenetic studies of monozygous twins is encouraged.

Amniography was employed to detect scar defects in the uterine wall of 141 near term gravidas before repeat cesarean section. Uterine defects were identified in only 2 patients; no defects were found in the amniograms or at cesarean section in the remaining 139. The value of amniography for detecting uterine defects is discussed.

The effect of fetal head compression on heart rate, cerebral metabolism and function was studied in 30 sheep experiments. Cerebral oxygen consumption decreased markedly as cerebral blood flow was significantly impaired by the increase in intracerebral pressure and vascular resistance. A fetal bradycardia or tachycardia occurred in 76% of the experiments. The fetal EEG showed a drop-out of faster frequencies, slowing and a decrease in amplitude prior to the onset of an isoelectric (flat) stage. All parameters recovered rapidly after the episode of cerebral ischemia caused by head compression.

Serum protein-bound fucose rose significantly in a high percentage of patients with advanced cervical and ovarian cancer, whereas no increase was found in patients with benign gynecologic disorders. Serum fucose levels were not influenced by precedent therapy and correlated well with the clinical stage of squamous-cell carcinoma of the cervix. This correlation was found to be exceedingly helpful in establishing the presence of and in evaluating the extent of recurrent cervical cancer.

To date, the histogenesis of vulvar Paget's disease remains unresolved. New electron microscopic findings, in 2 of the 5 cases presented in this report, demonstrate that the Paget cell may differentiate along either squamous or secretory lines and thus be found in the epidermis or within the glandular adnexa. The prognosis of Paget's disease, when confined to the vulva, depends not on the site and extent of the epithelial involvement but on the presence or absence of subjacent carcinomata. While considered to represent a form of carcinoma in situ of the vulva, Paget's disease itself does not become invasive. Experience with topical 5-FU in the control of local recurrences after surgery is encouraging and offers a new approach to therapy.

Forty patients with vulvar melanoma were registered at the MD Anderson Hospital and Tumor Institute from March 1944 to April 1970. The 30 previously untreated patients were retrospectively staged and the results of treatment evaluated. The gross 5-year survival rate among 14 eligible patients was 50%. Prognosis is related to stage. Clinically positive regional nodes is an ominous finding. Radical vulvectomy and bilateral groin node dissection constitute the minimum treatment. In most cases bilateral pelvic lymphadenectomy should be included. The vaginal-urethral margin of resection is the most frequent point of failure with this management.

Vaginal bleeding was the only presenting symptom in a patient with adenocarcinoma of the urachus. This case report is presented along with a review of the literature and the embryology and anatomy involved. Special emphasis is placed on the poor prognosis of these tumors and the lack of a uniform mode of therapy.

One hundred and thirty-eight Pomeroy tubal ligations were performed during the early puerperium under local infiltration anesthesia. Optimal intravenous premedication was achieved with 20 mg of Nisentil and a total dose of Valium based on 0.3 mg/kg of body weight. Delays caused by operative scheduling were avoided by performing the sterilizations in the delivery room. The technic was well accepted by patients and is now employed routinely.

Vaginal hysterectomy not only offers complete protection against childbearing but prevention from future benign and malignant uterine disease. This concept has been advocated and practiced by some gynecologists in the past, but more vaginal hysterectomies have been performed for sterilization under the guise of mild, asymptomatic, benign uterine disease. Vaginal hysterectomy for sterilization was performed by 9 fully trained gynecologists from February 1, 1970 through June 30, 1971. The averages and ranges for age, parity, operative time, hospital stay and estimated blood loss are listed and discussed. Operative and postoperative complications, additional surgical procedures, pathology findings and contraceptive status of these patients at the time of operation are presented and discussed. Also presented and discussed is 1 operative death. Our data reinforce the concept that vaginal hysterectomy is the preferred method of sterilization in the young female who has completed her family and wants to be protected against future benign and malignant uterine disease and be spared the drudgery of menses for a 15–20-year period.
