One of every five pregnancies in Trinidad and Tobago occurs in a grand multipara, a woman who has previously had at least six gestations terminating beyond Week 28. Several factors contributing to this high incidence of excessive parity are observed to include a lengthy reproductive career, illegitimacy, lack of religious beliefs, ethnic differences, and many ephemeral psychosocial influences.

Among 3060 cesarean sections, 560 were performed upon 123 women who had individually experienced from 4 to 8 cesarean deliveries. There was only 1 neonatal death and no maternal death. Three uterine ruptures occurred, all in classical section scars. Nine of the 14 eventual cesarean hysterectomies were performed for thin scars which could doubtless have been repaired.

Two PKU patients (sisters) were studied throughout the course of pregnancy. Microcephaly and heart disease were present in both infants. Cord blood phenylalanine levels were higher than simultaneous maternal levels. The infant that survived rapidly cleared the excess phenylalanine, with blood concentrations returning to normal within 24 hr. A phenylalanine loading test showed a normal phenylalanine curve, and the fasting phenylalanine- to-tyrosine ratio was within normal limits. The other infant died during the newborn period because of pulmonary Iymphangiectasia and multiple cardiac anomalies. Of 26 previous pregnancies in these 2 women, 16 had resulted in spontaneous first-trimester abortions; all infants carried to term had intrauterine growth retardation and microcephaly. Cardiac malformations were present in 9 out of 12 term infants, including the latest 2 reported here.

Amniocenteses were performed upon gravidas ranging from gestational weeks 29—43 to determine the amniotic fluid osmolality of normal and abnormal pregnancies. The osmolality decreased with advancing gestational age and continued to do so after the estimated term date. An amniotic fluid osmolality level of 250 mOsm/liter or less is suggestive of fetal maturity. Amniotic fluid osmolality may have potential value in predicting the onset of labor.

On the basis of the clinical study of 720 obstetric patients and an experimental study of tissue cultures, it is concluded that ultrasonic Doppler inspection of the fetus, as presently practiced, is not hazardous to mother or fetus.

Observations were made of 736 fresh abortuses obtained from therapeutically interrupted pregnancies of approximately 30—160 days ovulation age. Sex chromatin tests in all conceptuses and morphologic studies of gonads and external genitalia whenever possible revealed that the lower the sex ratio, the younger the embryonic age. The importance of standardization of a simple technic for determining the sex of embryos is discussed in detail, enabling comparison with earlier investigations.

A modification of postoperative suprapubic drainage of the bladder by means of a bulbtype retention tube (Foley catheter) is described. This easily executed method provides patient comfort, reduces incidence of urinary tract infection, and hastens recovery following vaginal plastic procedures.

We are gathered as doctors, doctors who have dedicated their careers to the care of the female. Thirty years ago Sigerist said: “From a private relationship between two individuals, medicine is rapidly becoming a social institution. It is one link in a great chain of social welfare institutions. Medicine, usually regarded as a natural science, actually is a social science because its goal is social. In combatting disease the physician uses methods of natural sciences every day, but to social ends.” And even centuries earlier, Matthew, in answer to Pharisees who desired him to show them a sign from Heaven, “Ye can discern the face of the sky; but ye cannot discern the sign of the times?” (St. James version). He surely made a point. We have not fully discerned the sign of the times.
