摘要:
Anticonvulsant therapy during pregnancy may result in fatal bleeding in the neonate because of severe depression of vitamin K-dependent coagulation factors II, VII, IX, and X. The clinical course and management of one such case of hemorrhage recently encountered by the authors are discussed.

摘要:
Pregnancy in patients with periarteritis resulted in maternal death in 7 of the 8 cases that have been reported. In the present case, periarteritis was in remission throughout the pregnancy; the patient was thus the second known maternal survivor. The infant also did well. It is suggested that pregnancy probably does not have as direct an effect on the course of the disorder as appears from the outcome of the previous cases. Extreme caution must prevail, especially as diagnosis is often difficult and experience so limited.

摘要:
A patient with 46,XY karyotype and 17a-hydroxylase deficiency is reported who illustrates marked virilization of the external genitalia. Marked phallic development and almost complete labioscrotal fusion with no development of a utriculovaginal pouch were noted. Because the perineum was essentially similar to that seen in male-to-female transsexuals, vaginal construction required the Mclndoe procedure. Hence, although some patients with 17α -hydroxylase deficiency have minimal virilization of the external genitalia, this patient's history indicates the operative management'necessary in the opposite extreme, when patients show complete masculinization. Hormonal evaluation after gonadectomy revealed the enzyme deficiency based on abnormalities of steroid secretion by the adrenal cortex.

摘要:
Two cases of herpetic endometritis are reported. In both patients, the disease developed from documented herpetic cervicitis via transcervical ascension. One patient recovered spontaneously. Generalization and death occurred in the other patient, a renal transplant recipient treated with immunosuppressive agents. The histologic features of herpetic endometritis are illustrated. Intranuclear inclusions and ground-glass nuclei were found in endometrial glandular and stromal cells. Focal necrosis was prominent. Herpetic virus particles were demonstrated by electron microscopy in both cases. Ascending herpetic endometritis is a rare complication of a common disease. In patients with deficiencies of the immune system, herpetic endometritis may represent the initial step toward dissemination and warrants particular attention.

摘要:
A case of herpes simplex virus (HSV) introduced during the intrapartum period by a fetal scalp electrode is reported. This occurred in a mother with no past history of HSV infection who was transferred from a primary to tertiary care hospital with premature rupture of the membranes at 34 weeks' gestation. Herpetic lesions began at the site of electrode application on the sixth day of life, and later became disseminated. Type 1 HSV was the agent identified that caused disseminated fatal neonatal disease.
