The clinical course and histologic features of 118 granulose cell tumore adn 82 theca cell tumors were reviewed. Al-though the 2 cell types are related, important differences exist in their behavior. Theca cell tumors (TCTs) are virtually benign, while granulosa cell tumors (GCTs) exhibit less malignant activity than most ovarian carcinomas. Characteristically detected at an early stage, GCTs may recur locally year after the initial diagnosis. survivial figures depend on tumor stage. Recurrence of GCTs is associated with a high mortality rate. The cornerstone of treatment is total abdominal hysterectomy and bilateral salpingo-oophorectomy, regardless of age. Adjuvant radiation or chemotherapy may be helpful when lesions are higher than stage Iai. Therapy for recurrence usually requirs operative execision combined with radiation, chemotherapy, or both.

Two cases of gastrointestinal tract injury associated with extrauterin intrauterine devices (IUDs) are reported and compared with the 6 previously reportd cases. Puerperal insertion was a factor in 6 cases, but 5 of these patients developed gastrointestinal symptonms only after a subsequent labor and delivery. Significnat morbidity was present in most patients, and the diagnosis was often delayed and difficult to establish. Laparotomy was required in 7 cases. An extrauterine device should be removed promptly to minimize the risk of developing serious gastrointestinal complications.

Three cases of mucinous carcinoma of the endometrium are presented. These can be distinguished anatomically from endocervical carcinomas and histochemically from endometrial clear cell carcinomas. Clinically these patients do not differ significantly from othere with endometrial carcinoma, but he tumore produce large amounts of extra-cellular and intracellular neutral and acid mucopolysaccharides. Mucinnous carcinoma is regarded as a morphologic variant of endometrial carcinoma.

A white primigravida with acute toxemia and hydramnios delivered a hydropic male infant who dies. A neuroblastoma of the right adrenal gland was found with metastatic spread to the liver and other organs. The placental capllaries contained tumor cells. Pathogenesis of hydrops, hydramnios, and maternal toxemia and the consequence of tumor cells in the placenta are discussed.

This report describes a patient with Von Willebrand's disease whose second-trimester abortion was complicated by hemorrhage. Factor VIII levels were low and the patient responded well to factor VIII transfusion. The authors believe that abortions can be performed safly in patients with Von Willebrand's disease. If factor VIII is low, prophylactic cryoprecipitate is indicated.

Real-time sonography was used to evaluate 158 patients with threatened abortion. Fetal motion was first detected during the seventh gestational week and with increasing frequency thereafter in 73 patients with viable preganancies continuing to term. Only 2 of 65 patients who aborted demonstrated featal motion. The presence or absence of fetal motion was most reliable after 7 weeks' gestation for establishing a prognosis for a given pregnancy. Seventy-two of 74 pregnancies with fetal motion continued to term, whereas 63 of 64 pregnancies without fetal motion aborted. A method for using real-time sonography in the management of threatened abortion is presented.

A simple, rapid, economical, and accurate test to assess fetal pulmonary maturity on a 24-hour basis, 7 days a week, is urgently needed. A 15-minute test has been developed that correlates well with fetal pulmonary maturity.Optical dinsity (OD) readings at 650 nm of ≥ 0.15 in pigment-free, centrifuged (2000 xg, 10 minutes) amniotic fluids obtained from gestations of 33 to 42 weeks correlate with fetal pulmonary maturity (131 of 131, 100%). When OD readings are < 0.15 in fluids from gestations of 24 to 40 weeks, and antepartum steroids are utilized, the incidence of respiratory distress syndrome (RDS) s 8.3% (14 of 169). The true false-negative rate in this group is therfore unknown.

The fetal heart rate (FHR) response to sound stimulation has been studied. The normal fetus exhibits an accleration of at least 15 beats/min sustained for at least 2 minutes. This response was studies in 116 pregnancies in which the fetus was considered to be at high risk. The response was impaired in the 3 cases of perinatal death, in 14 of 19 cases of perinatal morbidity, and in 17 of 27 fetuses whose subsequent birth weight was below the fifth percentile. An impaired response was of greater predictive value than the nonreactive pattern of nonstresed FHR monitoring.
