Artificial abortions were performed in 7 midterm patients by inducing mechanical stimulation of the uterus by placement of laminaria tents in the cervical canal and a metreurynter into the lower uterine cavity to give a mechanical stretch. The changes of uterine contractility during the treatment were monitored by intraamniotic pressure recording. The intraamniotic pressure during the labor was usually within 50 mmHg, with a frequency of 2-5 times per 10 minutes; the resulting uterine activity (intensity X frequency) being 100-200. The fetus was aborted unharmed in all cases. The delivered placentas were histologically normal throughout their thickness (amnion, chorion, and decidua). Some theoretic considerations are given on the mechanism of onset of labor induced by mechanical uterine stretch.

A total of 1579 Rh-negative women who were delivered of Rh-positive babies received Rh0(D) immune globulin (human) or Rh0(D) immune globulin (human) PPF to protect them against sensitization by the Rh factor. Follow-up serologic evaluations were carried out 6 weeks, 4 months, and 6 months following delivery. Efficacy of treatment, as demonstrated by antibody titer data after 6 months, was 99.5% in women with an ABO compatible pregnancy and 100% in other women.

An unregistered patient in the immediate puerperium developed a bizarre fever. Prolonged work-up demonstrated acute hematogenous tuberculosis with probable placental transmission to the fetus. The changing patterns of tuberculosis, and the diagnostic and therapeutic problems presented by miliary tuberculosis in late pregnancy are discussed.

HCS levels in venous blood collected from 30 preeclamptic and 30 normal patients during the latent phase of labor were determined using a radioimmunoassay technic HCS levels in cord blood were determined on all 60 patients. A small decrease in maternal HCS values with a marked elevation of fetal HCS was found to be present in the preeclamptic population.

Human placental lactogen (HPL) levels were estimated before and during labor in normal pregnant subjects. There was no change in the circulating levels of HPL in the 14 days preceding the onset of labor. During labor, there was a small, progressive fall which may reflect uteroplacental ischemia associated with uterine contractions. No relation was found between HPL levels in the last weeks of pregnancy and the gestation at which spontaneous labor occurred. It is not possible to attribute any direct role to HPL in the initiation and maintenance of labor.

The spiral electrode greatly simplifies direct fetal heart rate (FHR) monitoring. The technic of application is easily learned, and no accessory instruments are needed. Overall function and reliability are superior to the silver-silver chloride clip type electrode. Detailed evaluation of 110 patients is described as well as the clinical trial in more than 2000 patients.

Between July 1, 1970, and June 30, 1972, 3542 first-trimester and 876 second-trimester pregnancies were terminated at City Hospital Center, Elmhurst, New York. The present rate of abortion is 1009 per 1000 deliveries. No maternal mortality occurred in this series. Maternal early morbidity rates have been 4.7% for first trimester and 15.1% for second trimester abortions. Salutory changes have been noted in other health care services. The incidence of delivery without prenatal care has decreased by 44.4%; of immature delivery, by 23.1%; of premature delivery, by 16.5%; and the corrected perinatal mortality rate, by 29.2%. Antepartum clinic utilization increased. There has been significant elimination from the child-bearing population of certain categories of “high risk” obstetric patients—the very young, the elderly, the unmarried, and the emotionally disturbed. The spontaneous abortion rate has decreased by 52.2%, and “septic” abortions have been almost entirely eliminated. Benefits observed in other aspects of health care are: pregnancy testing and counseling; gynecologic cancer detection and cytologic screening; venereal disease screening; sickle cell trait/disease screening; and detection of intercurrent medical, surgical, gynecologic and psychosocial problems. The abortion program has served the community not only by the solution of immediate health crises, but as a major portal of entry into a comprehensive health care program.
