In 6 female patients with urinary incontinence who underwent a detailed urodynamic evaluation, the urethral pressure profiles assumed a characteristic biphasic shape. The biphasic pressure profile was in sharp contrast to the nearly bell-shaped curve seen in normal patients. In these 6 patients, a significant decrease in the urethral pressure was observed at the anatomic location of the diverticular orifice. This study assisted in the diagnosis of urethral diverticula, which was later confirmed by urethroscopy and positive-pressure urethrography.

Bilateral renal agenesis (Potter syndrome) in the neonate is incompatible with life. Unfortunately, these infants are often delivered by cesarean section, as premature labor and breech presentation are commonly associated with the condition. In reviewing the obstetric course of 6 women who recently delivered such babies, the authors have found that antepartum diagnosis is possible in most cases. Specifically, vaginal delivery should be strongly considered for patients in premature labor with intact membranes and breech presentation when there is ultrasound evidence of an abnormally shaped fetal head, absence of the fetal bladder, and severe oligohydramnios.

The increasing incidence of multiple gestations and the need for genetic diagnosis often pose a question as to the safety of amniocentesis in these cases. Under careful sonographic control and using indigo carmine as a marker, a technique for amniocentesis in a triplet gestation is presented.

Several cases of benign enlargement of neurofibromata during pregnancy are reported in the literature. We believe the present case to be the first report of malignant schwannoma occurring in such a tumor during pregnancy. As this case illustrates, malignant sarcomatous change must be carefully considered in such patients. Prompt diagnosis and radical therapy offer the only chance of survival in malignant schwannoma.

A case of large cell undifferentiated carcinoma of the lung metastatic to the placenta and a review of the literature are presented. This is the 44th documented case of placental and/or fetal metastasis from maternal cancer in the past 113 years. Fifty percent of these cases are either malignant melanomas or hematopoietic malignancies. Evidence indicates that these 2 malignancies are more likely to metastasize to the products of conception than are other malignancies.

A young patient with a prolactin-secreting pituitary microadenoma, galactorrhea, low estrogen levels, and a welldifferentiated adenocarcinoma of the endometrium is reported. The association of hyperprolactinemia and adenocarcinoma of the endometrium has not previously been described.

Acute febrile neutrophilic dermatosis, otherwise known as Sweet syndrome, usually occurs in women after prior respiratory infections. The first case of Sweet syndrome with involvement of the female genital tract is reported here. The woman recovered completely, and no evidence of other underlying disease has appeared.

Previous reports have implicated the Dalkon shield intrauterine device (IUD) in septic second trimester abortion and maternal death from sepsis. In the case reported here, Fusobacterium necrophorum, a rarely pathogenic vaginal anaerobe, gained access to the uterus in a woman wearing a Dalkon shield and caused acute parametritis, overwhelming peritonitis, systemic sepsis, and death. It is well known that IUDs can contribute to the development of serious pelvic infections, and in this case it is possible that the Dalkon shield was the cause of the ascending infection into the uterus.

In a previous case report, intrauterine fetal tachypnea was associated with sudden fetal death. The authors have observed a similar case of fetal tachypnea. Early intervention occurred and a normal fetus was delivered. Pathophysiologic mechanisms for the phenomenon are discussed.
