The clinicopathologic findings of 13 patients with ovarian pure germinomas (dysgerminomas) were studied to assess modes of therapy. The patients' ages ranged from 12 to 40 years. Ten patients had unilateral tumors clinically confined to the ovary (Stage I), including 1 patient who had an unsuspected microscopic germinoma in a normal-appearing ovary. Of those with Stage I tumors, only 1 patient developed metastases and died; however, the presence of other elements in the metastases suggested that her primary ovarian neoplasm actually may have been a malignant mixed germ cell tumor. The other 9 patients are alive without tumor after postoperative intervals of 0.3 to 29 years. Three patients had more advanced lesions (Stages II–IV) and 2 died of metastases following orthovoltage irradiation. The results of this study support the concept of individualized therapy for patients with ovarian germinomas.

Cell-mediated immune response was measured in 23 patients with ovarian cystadenocarcinoma, 38 patients with benign ovarian tumor, and 44 healthy volunteers. The method used two indexes: the lymphocyte response per unit volume of peripheral blood to phytohemaggluttnin (PHA) and the immunosuppressive effect of serum on the response of normal lymphocytes to PHA stimulation. The lymphocyte response per 50 μl peripheral blood did not differ significantly between patients with ovarian cancer and healthy volunteers. The serum effect, in contrast, differed significantly between malignant and benign ovarian tumors, and was found to increase significantly even when the cancer masses were as small as about 5 X 5 X 5 cm in size, ie, in FIGO Stage 1. It is our belief that the measurement of the serum effect in patients with any ovarian tumor enables the early detection of ovarian cancer.

A group of prostitutes had cultures taken and were interviewed to determine rate and risk factors for gonorrhea infection in a weekly screened prostitute population in Taiwan. All cultures were taken from the endocervix by sterile swab during a speculum examination and immediately plated on Thayer Martin media. Positive results were confirmed by Oxidase reaction and sugar fermentation. Forty-three of 515 known cases were positive (8.3%). This is felt to represent a reinfection rate as all participants are screened weekly and promptly treated if positive. Age, douche frequency, and pregnancy history did not differ significantly among the positive and negative culture groups. Increasing frequency of intercourse per day did increase the chance of infection. Infection rate decreased with length of employment. Positive culture rates associated with various methods of contraception were as follows: oral contraceptives 7.0% (315), IUD 8.3% (24), condom 21.1% (33), diaphragm 0% (17), surgery 0% (5), and none 10.1% (118). The risk of gonorrhea infection in a prostitute population is directly related to the exposure rate during the period between screening. The use of oral contraceptives did not increase the risk of infection. The role of mechanical barriers in the prevention of gonorrhea infection needs further study.

The endodermal sinus tumor of the ovary is a rare and highly lethal germ cell tumor. At least 10 different names have been applied to it, and so long as its name remains unsettled the collection of information will continue to be greatly impeded. One of the patients who came under our observation died after 21 months despite surgical removal of all evident disease followed by radiation and chemotherapy; the other patient, whose tumor could be removed only partially, and who received intensive chemotherapy for 4 months but refused all treatment thereafter, is well without evidence of disease 3 years after the initial diagnosis was made. Despite the dismal outlook for patients with this tumor, a few encouraging features emphasize the need to report new cases as they appear, and to continue the search for agents that will control it.

Renal and urinary tract anomalies have been reported to occur with a number of chromosomal abberations and a variety of müllerian developmental anomalies. Historically, the XY female has been privileged to remain free from these associated urologic complications. Presented are two types of the XY female in whom a major renal anomaly was found. It is possible that a potentially more frequent association of developmental anomalies has been overlooked. The urinary systems of XY females, when identified, should be investigated.

The average length of the umbilical cord at delivery is 21 ± 1 in., but lengths vary from 10 to 50 in. When distended and carrying blood between the fetus and the placenta, the cord is longer; the distended arteries and vein are spiraled about each other. No physiologic data explain the manner by which blood flows from the placenta to fetus. When pressures (60–80 mmHg in arteries; 10–20 mmHg in the vein) are recorded simultaneously, arterial pulse pressure has been observed to rise and umbilical vein pressure to fall. They are 180 degrees out of phase. When necessary physical factors are taken into account, it is found that the system satisfies all requirements of a pump known as one of the pulsometer type. This system is one that is widely used by engineers, but it is unique among physiologic systems.

The several components of a clinical clerkship were analyzed and correlated with Part II National Board scores and career choice. No positive correlations could be made between fulfilling educational objectives, utilization of audiovisual aids, general satisfaction with the clerkship, and eventual career choice or performance on National Board Examinations.
