The purpose of this study was to establish I) the sequential changes in specific activity of phosphatidic acid phosphohydrolase (PAPase) in amniotic fluid and its relation to the Iecithin/sphingomyelin (L/S) ratio; and 2) the origin of amniotic fluid PAPase. The increase in the amniotic fluid L/S ratio is preceded by an increase in PAPase activity, rising from 15 nmoles of phosphate released per milligram of protein per hour at 30 necks la 101) nmoles at 37 weeks. The mean PAPase activity in the nasopharyngeal fluid of the infant is 456 nmoles of phosphate released per ml per hour, the amniotic fluid mean PAPase activity at delivery being 129 nmoles ( P<0.01). These findings are consistent with the view that amniotic fluid PAPase originates, in part, from the fetal lung and likely participates in the regulation of the synthesis of lecithin.

Six hundred consecutive patients received intraamniotie prostaglandin F2α, to induce midtrimester abortion. The PGF2αwas administered by a general population of gynecologists according to a standard protocol. The aim of the study was to observe whether results would be similar to those reported in various research studies. Of 600 abortions, 460 were complete, 140 were incomplete, and there were no abortion failures. Other parameters were also similar to previous research results, indicating that PGF2αcan be an effective and safe means of inducing midtrimester abortion when administered by the practicing gynecologist.

From June 1973 to October 1974, 221 patients underwent menstrual induction in the private offices of a University Medical Faculty Group located in its major leaching hospital nith ready access to operating room facilities. The rationale for subdassifying early abortion as “menstrual induction” is discussed. In this series menstrual induction was performed on patients who were 7 to 21 days beyond the date of an expected menses. Histologic examination of the tissue removed was consistent nith a diagnosis of pregnancy in 98.2% of the patients. Complication rates following the use of the practices outlined in this report were acceptably low.

In the field of maternal and child health the concept of the highrisk pregnancy has been used in many studies. However, it is still beset by problems that must be solved if any real progress is to be made. The ideal of a simple, reliable screening technique has not yet been achieved. Present methods are not only complex, they are not well validated. High-risk factors must be quantified if the values of various regimens of obstetric care are to be measured and if prognostic success is to be attained. Moreover, efforts at prediction should not cease with the current pregnancy but should extend to subsequent reproductive performance. This long-range approach to prognosis could not only improve patient care during the current pregnancy but could also become an essential factor in achieving optimal results throughout reproductive life. An extensive bibliography of studies of high-risk pregnancies is presented.

Dilatation and curettage ur hooking an occult intrauterine contraceptive device (ILID) may fail to recover the device on occasion. Theoretically, endoscopic observation should enable the controlled removal of this type of device. Of 200 patients who underwent hysteroscopic examination, 10 had an occult 1UD. In S cases, the device was visible through the hysteroscope, and in 6 patients a successful removal was accomplished with this endoscopic technic. Failures and problems are discussed, and the importance of adequate preoperative study is stressed. Hysteroscopy is an effective procedure in the recovery of the occult intrauterine contraceptive device in selected nonpregnant patients.

Disturbances in the blood coagulation mechanism arc seen by the obstetrician and gynecologist as rare complications of abruptio placentae, retained dead fetus syndrome, amniotic fluid embolism, toxemia, saline amnioinfusion, and septic abortion. Two cases of disseminated intravascular coagulation complicating gynecologic malignancy arc presented. Laboratory studies showed thrombocytopenia, hypofibrinogenemia, and increased fibrin degradation products. Derangements of hemostasis in patients with malignancy arc discussed from a clinical viewpoint.

Two cases of large bowel perforation by intrauterine contraceptive devices are presented. Both the Lippes LoopTMand Dalkon ShieldTMhave the potential of evoking omental adhesions anil causing bowel perforation. These serious complications justify the immediate removal of a contraceptive device from the peritoneal cavity by either laparoscopy or laparotomy.

A potent topical anesthetic drug, 1% telracaine, and a placebo were evaluated in a “double-blind” study to determine whether or not topical anesthesia could be achieved on the exocervix, endocervix, and endocenix cavity. No anesthetic effect of lelracaine was demonstrated.

Ultrasound methods for detecting fetal life during early pregnancy arc reviewed. By using recently developed techniques, fetal heart function can be detected as early as the 44th to 45th day of amenorrhea. Fetal movements in the amniotic cavity can be visualized from the 10th week on. If no signs of fetal life can be detected by ultrasound examination by the 10th week, the pregnancy is in jeopardy. However, the prognosis is favorable in over 90% of those cases of threatened abortion in which fetal life has been confirmed by ultrasound.

Physiologic and pathologic production of milk involves complex relations between the mammary glands, hormones, and the central nervous system. In all the galactorrhea syndromes there is a functional or mechanical problem at the pituitary level, with abnormal secretion or reserve of prolactin secretion. Stimulatory agents of prolactin, like thyrotropin releasing hormone (TRH), chlorpromazine, amino acids, and insulin, can be helpful in the study of the hypothalamic pituitary functional reserve, while the osmotic tests seem to provide a clear distinction between functional and tumoral causes. The inhibitoryagents of prolactin secretion, L-dopa and CB 154, permit the study of the negative control of the hormone. In addition, CB 154 appears to be an effective treatment for functional galactorrhea. hyperprolactinemia appears to exert an inhibitory influence on gonadotropins. Clomiphene, acting on the hypothalamus, and LHIRH, acting on the gonadotropes, permit the assessment of the gonadotropic hypothalamic-hypophyseal axis.
