Estradiol 17-β, the primary estrogenic hormone elaborated by the human ovary, though available in pure form, has not been used extensively in oral menopausal therapy because it is considered ineffectual due to poor gastrointestinal absorption. Other estrogen preparations in current clinical use are effective orally but are attended by drawbacks such as unpleasant side effects, high cost or suspected thromboembolic risk. A new dosage form of pure 17-β Estradiol, micronized for better absorption, was not only well tolerated but also clinically effective in this preliminary study of menopausal therapy.

Studies of the coagulation and fibrinolytic enzyme system in menopausal women on estrogen replacement therapy revealed a statistically significant increase in concentrations of fibrinogen and α1-trypsin inhibitor. A decrease in plasminogen was noted, using a fibrinolytic assay and antithrombin III activity (assayed by a coagulation technic). There was no change in Factor VII, X or plasminogen by radial immunodiffusion, α1-macroglobulin, plasmin inhibitor, platelet number and platelet adhesion. Fibrinolysis breakdown products and fibrin monomers were not detected. The present study supports the assumption that estrogen is the effective hormone to initiate changes in the coagulation and fibrinolytic enzyme systems in women taking oral contraceptives.

Thyroid function was evaluated in a group of 53 patients who had taken oral contraceptive pills for an average of 153 consecutive cycles. We found the same alterations reported previously by other authors in the PBI and T3. test. Values for131I in 40% of the patients were abnormally low, a finding that has not been reported previously. Levels of total thyroxine and free thyroxine (not index) were relatively unchanged. On the basis of this study, we can say that although individual tests show alterations, none of the patients was considered to have a thyroid disease. There is no evidence to indicate a definite relationship between prolonged use of oral contraceptive medication and the development of thyroid disease.

The proximity of the ureters to the uterine artery and other vital pelvic structures is a potential cause of operative complications. From July 1965 to July 1970, 617 vaginal hysterectomies were performed at the University of Kentucky Medical Center. Twenty-seven of these patients had postoperative intravenous pyelograms, and abnormal findings related to surgery were found in 5. One patient had unilateral ureteral ligation and a subsequent ureteroneocystotomy. The indications for postoperative intravenous pyelography are reviewed.

The true incidence of hydatidiform mole formation subsequent to clomid therapy is unknown. This association has appeared only twice in the published literature. Furthermore, the manner in which physicians prescribe clomid for the treatment of infertility is uncontrolled. A case report is presented to illustrate these two points.

A 20-year-old white female with hirsutism, oligomenorrhea and ovarian enlargement was studied using differential stimulation and suppression of adrenal cortex and ovaries. Although baseline urinary 17-ketosteroid levels were elevated, the patient exhibited normal adrenocortical suppression and stimulation and no increase in urinary 17-ketosteroids during the administration of dexamethasone combined with HCG. At laparotomy a benign cystic teratoma involving the entire left ovary was found. The right ovary had a characteristic polycystic appearance. A left oophorectomy and wedge resection of the right ovary were performed. Cyclic menses with biphasic temperature curves occurred for the first time after surgery, and pregnancy was established in 8 months. The patient's atypical response to dexamethasone and HCG and the possible role of the teratoma in the pathophysiology of her hirsutism are discussed.

Adenofibrocarcinoma of the ovary is quite rare. Although 17 cases have been mentioned in the literature, some of which may reflect different entities since clinical and pathologic discussions with followup have been found only in a few isolated instances. We have reported 4 patients who showed atypical and frankly carcinomatous changes involving the glandular epithelium of adenofibromas. In none was there evidence of metastasis or local spread. This neoplasm is frequently asymptomatic, growing quite large before discovery.

During medical examinations, the sexual feelings of the student-physician may be evoked. These feelings per se are normal. They become detrimental only if they are a) acted on—in the form of subtle seduction—or b) responded to with guilt—possibly by avoiding examinations that, although part of complete medical care, have sexual overtones.
