Termination of pregnaNC.y during the second trimester was accomplished safely in 309 women by means of intraamniotic instillation of a 23%-saline solution. Identification of the early warning symptoms of intravascular saline injection by careful attention to the alert, instructed patient during the solution's slow administration permits immediate discontinuaNC.e of the injection as well as rapid institution of measures to prevent anticipated CNS reactions. Intravenously administered oxytocin at the end of the latent period shortens the period of painful labor.

Determination of the creatinine content of amniotic fluid of 124 gravidas showed iNC.reasing values as gestation advaNC.ed. A creatinine level of less than 1.5 mg/100 ml correlated well with a gestational length of less than 37 weeks and a level of more than 2 mg/100 ml pointed to pregnaNC.y of more than 37 weeks. In addition there was fair statistical correlation between birth weights and amniotic fluid creatinine values. Infants associated with prior creatinine values of 2 mg/100 ml or greater were physiologically mature in their nursery behavior.

Experiments with an acrylic model of the intervillous space supplied by two individual inflow channels gave evideNC.e that mixing of the fluid in both areas took place rapidly unless they were separated by an intact waterproof baffle. However, if simulated cotyledons (moss) filled the two spaces, no separating baffle was required to prevent mixing. It is implied that if the model represents conditions obtaining in the human placenta, each cotyledon and its corresponding maternal bed constitute an independent circulatory unit.

This review of 28 pairs of twins, vaginally delivered to Rh-sensitized mothers, uNC.overed variable degrees of erythroblastosis, unrelated to the uni- or diovular character of the twins. The degree of hemolytic disease also varied when both twins were Rh positive, supporting the viewpoint that factors other than the maternal antibody titer affect fetal involvement.

An 85% return rate of vaginal irrigation smears, 93% of which were technically satisfactory, from 2867 women who recently had been subjected to cervical-scrape smears permitted comparison of the two methods. There was a similar false-negative rate (12–18%) with each type of smear. Performing four-quadrant cervical biopsies upon 86 patients (2.2%) who had at least Class III smears, the authors uNC.overed 5 instaNC.es of severe dysplasia, 24 instaNC.es of in-situ carcinoma, and 5 instaNC.es of minimal or questionable invasion.

CoNC.entrations of human growth hormone in the plasma during a 6-hr GTT of patients with several types of galactorrhea were shown to be no different from those of control subjects. Three patients with Thorazine-induced galactorrhea did have a comparatively attenuated response of growth hormone. The findings imply that abnormal secretion of growth hormone is not implicated in galactorrhea, and also support the viewpoint that prolactin and growth hormone are separate entities.

A survey of the occurreNC.e of vaginal candidiasis among 1001 patients seen consecutively in a private gynecologic practice, employing Nickerson's Medium for diagnostic culture, uNC.overed an overall iNC.ideNC.e of 8.5%. The disorder was asymptomatic in nearly 25% of the affected patients. It was found but rarely among prepuberal girls and menopausal women. PregnaNC.y and the use of oral contraceptives iNC.reased the iNC.ideNC.e of vaginal moniliasis but not as significantly as did the use of antibiotics and cortisone.

A group of 479 estrogen-deficient women of menopausal age were treated with a form of orally administered conjugated estrogens. The author urges the caution of regularly scheduled examinations and diagnostic curettage for even the slightest uterine bleeding. The iNC.ideNC.e of breast and endometrial caNC.er was not iNC.reased in the estrogen-treated women when compared with that in the author's other patients.

Specimens of human endometrium, obtained by biopsy daring each third of the proliferative phase and each day of the secretory phase, were dated by histologic criteria and studied histochemically with the aid of electron microscopy for the detection of three phosphatases. Alkaline phosphatase, identified on epithelial cell membranes but not in nuclei or stromal cells, was present throughout the proliferative phase and into the midsecretory phase. Acid phosphatase was observed in lysosomes of glandular epithelium after Cycle Day 21. Glucose-6-phosphatase, recognized for the first time in human endometrium, was most evident during the time of ovulation and early secretory phase. The timed localization of the enzymes suggests that they are biochemical determinants of the intracellular actions required to support implantation and early growth of the coNC.eptus.

Intravenously administered diazepam was tolerated well by 94 parturients and their fetuses. Compared to a similar placebo-treated group, the diazepam recipients required significantly less meperidine during labor.
