Study of the chromosomal content of endometrial cells by means of three methods—direct squash, tissue culture squash, and air-dried cell suspension—showed consistent pattern of 46XX. Aneuploidy was associated with broken cells; it was not characteristic of endometrial cells associated with amenorrhea, habitual abortion, infertility of unknown origin, or abnormal hormonal state. Caution is urged in interpretation of aneuploidy, especially from squash preparations.

In an 8-year study of 18,378 consecutive obstetric patients, irregular antibodies were detected in 299 (1.62%). The incidence of anti-Rha(D) sensitization was 1.66%. The most commonly encountered irregular anti-bodies were: anti-rh´ (C), anti-rh (E), anti-Kell, anti-Lebanti-Lea, and “unidentified.” No neonatal death could be attributed to an irregular antibody alone. Most of the serious problems occurred with the irregular antibody in combination with anti-Rha(D). Routine screening of all Rh-positive and all Rh-negative patients is advisable to provide optimum obstetric care. A cord antiglobulin (Coombs') test must be done oh all patients to alert the clinician to the possibility of erythroblastosis fetalis and the blood bank to rare antibodies, obviating problems if the patient should require a blood transfusion.

Amniotic fluid samples (140 from 111 patients) were obtained by transabdominal or transvaginal amniocentesis and examined for “fat cell” counts and creatinine concentrations. Infant weights at birth were used as the measure of maturity. Fetal maturity was correctly indicated by 94% of the fat cell counts of 20% and greater, and by 97% of the creatinine concentrations of 1.5 mg/100 ml or more. Fat cell counts lower than 20% and creatinine concentrations lower than 1.50 mg/100 ml were less accurate in indicating a premature baby.

A patient with the complete form of the testicular feminization syndrome and 46,XX/46,XY mosaicism is presented. This is only the fourth reported patient with the testicular feminization syndrome and sex chromosome mosaicism. This unusual chromosomal pattern was not clinically expressed and was probably a chance finding. The mechanism which may cause an XX/XY mosaic pattern is discussed.

Two case reports are presented of infants with large sacrococcygeal teratomas obstructing labor after partial vaginal delivery. One child died, but the other survived following trochar aspiration of the cystic mass via abdominal hysterotomy. Proper diagnosis and management of dystocia caused by sacrococcygeal teratoma may save the infant's life.

A departure from the classical management of the pregnant diabetic patient was utilized during 4-years with 133 women, of whom 113 were gestational diabetics. Arbitrary delivery before term was not employed; each patient was seen weekly by an internist-obstetrician team. From Week 30, maternal 24-hr urinary excretion of estriol was obtained weekly and used as a guide to fetal well-being. Only 8 of the 113 gestational diabetics required preterm delivery because of falling estriol values; 16 of the 20 prepregnancy diabetics were delivered at 38 weeks or later. The overall perinatal loss was 2.6%; for the gestational diabetic group was only 0.9%.

In the placentas of 48 diabetics, gross lesions were uncommon, but inconsistent abnormalities of villous maturation were common. Principal histologic abnormalities seen in the diabetic's placenta were an obliterative endarteritis of the fetal stem arteries, thickening of the trophoblastic basement membrane, and villous fibrinoid necrosis; all may result from immunologic damage. Villous fibrosis and excess syncytial knot formation were frequently observed; they are probably secondary to the fetal arterial abnormalities.

The clinical significance of completely circumvallate and circummarginate placenta was determined by an analysis of the records of 39,514 obstetric patients and their offspring enrolled in the Collaborative Project The data refute previous studies attesting to the frequent occurrence and serious consequence of extrachorial placenta. Extrachorial placenta was found to be more common in multigravidas. White women with circumvallate placenta had increased rates of antepartum bleeding, prematurity, perinatal, infant, and child deaths, but the stillbirth rate was not augmented. Similar trends were noted in the Negro circumvallate group. Nevertheless, these complications could not be ascribed to abnormal placentation because such problems are more frequent in the black population for other reasons. Even complete circummarginate placenta was not serious clinically in either race.

The near-fatal courses of 2 patients with ruptured cornual pregnancy in a rudimentary half of the uterus dramatizes the danger of such an implantation. Elective removal of such a vestigial horn, when it is identified prior to gestation, seems a reasonable form of prophylaxis.

Tissue from the female genital tract was systematically examined for activator activity of the fibrinolytic enzyme system using the autographic technic of Todd. Endothelium-lined channels were highly active. Much activator was found in musculature of both corpus uteri and fallopian tube, as well as in the racemose glands of the endocervical lining. Comparatively little activity was found in the endometrium. It is irregularly present in proliferative endometrium but diffusely distributed in secretory endometrium. Stratified squamous epithelium and submucosal fibrous tissue found to be inactive.
