Studies to elucidate the mechanism responsible for tetracycline toxicity in pregnant women during an episode of acute pyelonephritis are reported. The rate at which tetracycline disappears from blood, the quantity excreted in urine, endogenous creatinine clearance, and liver function have been determined. From these and previous studies, it would appear that renal function impairment, which occurs during the course of acute pyelonephritis in pregnancy, is an important factor in the genesis of tetracycline toxicity.

To revitalize interest in pelvic pneumography among present-day gynecologists and radiologists, a series of 275 cases are presented with discussion of the technic, indications and contraindications. Results of the procedure, when compared with bimanual pelvic examination and laparotomy, show that the accuracy of diagnosis is improved by 15% when pelvic pneumography is used prior to laparotomy.

Rupture of an enterocele followed by evisceration of the bowel through the vagina is rare. This paper presents the case report of a 72- year-old patient who had had a vaginal hysterectomy that was followed 8 months later by rupture of an enterocele and evisceration of the bowel. In addition a brief review of the 15 previously reported instances is included.

Positive cultures were obtained in 67 of 377(179c)pregnant clinic patients screened for asymptomatic bacilluria.E coliwas present in the majority(58%).There was a suggested, but not significant, elevation of cord 7S anti-A and anti-B isohemagglutinin levels in infants of mothers withE Colibacilluria. Premature labor, premature rupture of membranes, toxemia, abruptio placentae and intrapartum fever were more prevalent among the same groups. Treatment of the asymptomatic bacilluria did not seem to affect fetal outcome or isohemagglutinin levels. There was a significantly higher incidence of chronic urinary tract disease among those patients with gram-negative bacilluria.

Nine patients with advanced carcinoma of the vulva which required exenterative procedures have been treated by the authors during the past 10 years. Four patients are still alive 45 months or more after treatment, although one has had a local recurrence. Only one operative death occurred; the four late deaths were all due to local recurrences. The rationale for ultraradical surgery' is discussed from the standpoint of the growth pattern of the cancer. Consideration is given to the possible origin of these slower growing tumors from granulomatous disease as well as to blockage of metastases because of obstructed lymphatic channels. A one-stage surgical procedure that includes bilateral groin dissection and a generous perineal phase is discussed in full as is the rationale for surgical treatment.

Diffuse pulmonary lymphangitic involvement secondary to cervical carcinoma is a rare form of pulmonary' metastatic disease. The clinical course is one of rapid deterioration with marked respiratory symptoms. The early Xray pattern resembles that of bronchopneumonia or congestive heart failure, with which the disease is often confused. The pathologic changes in the lungs include permeation of deep and superficial lymphatics giving a characteristic gross and microscopic picture.

A mesonephroma of the uterine cervix in a 14-year-old white female is reported. She was treated by Wertheim hysterectomy and pelvic lymphadenectomy with conservation of the ovaries. The histologic pattern, criteria for diagnosis, and clinical manifestations of mesonephroma of the uterine cervix are described and the prognosis is reviewed briefly.

Twelve serum proteins in amniotic fluids and in corresponding maternal sera from 18 Rh sensitized gravidas were studied by radial immunodiffusion. No changes in the concentration of any of the proteins were found at different gestational ages when concentrations were expressed in g/100 g of protein. However, in some severely affected patients, total protein was markedly increased.

Intrauterine pressure (IUP), fetal heart rate (FHR) and scalp pH, were monitored during labor in patients admitted to a research center and community hospital. Labor monitoring has practical limitations. The membranes must be ruptured and the cervix must be dilated 4 to 5 cm. One-half of the records cannot be interpreted, and 25% of the patients describe fear and pain associated with instrumentation. Ninety-two percent of all patients, however, liked the active medical and paramedical supervision of their labor. In a community hospital, monitoring is impractical without an around the- clock monitoring team. The main limitations of fetal scalp pH determinations are technical and time factors. It is concluded that the major contribution of present-day monitoring methods is the attention that the obstetrician and nurse focus on the fetus and uterus of high-risk patients.

In view of the increasing use of epidural analgesia during labor, attention is directed to the effects of this conductive analgesic on FHR and fetal scalp pH—eg. bradycardia and acidosis. These changes are more closely related to alterations in maternal blood pressure and the disorder produced in the uterine placental bed than to the direct effects of the analgesic upon the fetus. In most cases. FHR returns to normal spontaneously; in others, bradycardia persists until delivery. Changes in pH occur only with severe FHR abnormalities and, once an acidotic level has been reached, biochemical improvement is slow, even though maternal blood pressure and FHR have returned to normal.
