Karyopyknotic indices (KI) for 1454 normally menstruating women ranged from 1 to 84 at various times throughout the menstrual cycle. The highest mean value was 28 for smears taken on cycle days 11–15 from women 41–45 years old. Less than one fifth of all KI values were above 30. Because of enormous variations in individuals, as well as the cyclic pattern of estrogen production, single KI determinations for regularly menstruating women are not clinically useful. Smears from 1000 post-menopausal women not taking estrogen showed a progressive shift to an atrophic pattern (parabasal cells only) with advancing age, but 9% of women over 75 still yielded smears composed entirely of intermediate cells. Superficial cells could be found in about 25% of post-menopausal women, but their KI values were generally below 5. The significance of KI values above 10 in elderly women usually is obscure, although occasionally, this finding may be associated with ovarian cancer.

Three live, attenuated rubella virus vaccines (HPV-77 DK129HPV-77 DE and Cendehill) were given to 107 adult women within one week postpartum. Of these, 66 did not have rubella HI antibody before immunization while the remainder had titers of 1:10 or 1:20. Seroconversions were comparably high for each vaccine; there was no significant difference in the antibody levels attained. Joint pain was a common complaint in each group of susceptible women (40.9%). When assessed in terms of limitation-of-motion, swelling or duration, symptoms were most marked in DK12. and least severe among the Cendehill recipients. In one DK12recipient, recurrences were noted, the last 8 months after inoculation. Of the 66 women who were initially seronegative, 2, both DK12recipients, complained of joint symptoms severe enough to limit their ability to care for the newborn child. No detectable HI antibody was found in 18 infants from 4 to 11 months after the mother had been immunized.

Of 1884 patients delivered during a 10-month period, 236 (12.5%) were found to lack antibody to rubella when screened by the hemagglutination-inhibition method. Rubella immunization was offered to these women during the immediate puerperium, and the results in 201 patients are reported. In ninety-one percent of those vaccinated, seroconversion occurred. The incidence of side effects of all types was 22.1%, of arthralgias in particular, 17.8%. Vaccination-related symptoms were transient in all but two patients. No morbidity was seen in the infants of the vaccinees, nor was seroconversion found in this group. Postpartum rubella immunization seems to be an effective, well-tolerated method to eliminate from risk those women of childbearing age who are susceptible to rubella.

By including the hemagglutination-inhibition (HI) test as part of the routine laboratory procedure for prenatal patients, the authors found that 10.3% of 947 pregnant women were susceptible to rubella. These findings agree with those of others who have reported that the patient's history is not a reliable guide to her immune status. Since the HI test can be carried out in most laboratories, it should become a routine part of the prenatal examination. Rubella-susceptiblc women could be vaccinated with a rubella virus vaccine during the post-part um period. Consideration should also be given to vaccinating susceptible women of childbearing age who are not pregnant. Women in both groups must agree to avoid becoming pregnant for two to three months.

The ultrastructural morphology of a hydatidiform mole which subsequently progressed to metastatic trophoblastic disease is described. The fine structural observations were correlated with previously reported biochemical findings. The most paramount observation was the presence of increased amounts of lipid vacuoles within trophoblast. This ultrastructural finding suggested an accumulation of substrate occurred which could be the morphologic representation of unused steroid precursors.

Twenty-two microinvasive carcinomas of the uterine cervix are analyzed with regard to their histologic presentation and clinical data. The cancers are characterized by essentially identical morphologic features. The lesion identified in each instance is regarded as the incipient form of microinvasive carcinoma and may be differentiated from the advanced variant by a set of morphologic criteria rather than by the depth of stromal penetration. Follow-up observation supports the conclusion that incipient microinvasion does not involve lymphatic channels or blood vessels and may be treated in a manner similar to that for carcinoma in situ.

A case report of asymmetrical gonadal dysgenesis in a 5-year-old child demonstrates the diagnostic utility of gynecography in establishing the status of the internal genitalia prior to corrective surgery. Specific indications and details of methodology which will facilitate gynecography during infancy and early childhood arc discussed.

Chromosomal and electron microscopic studies of a chorangioma of the placenta are reported. The normal chromosome complements from the tissue cultures of the tumor were consistently observed and the ultrastructural features of the tumor were noted to be similar to those described for normal capillaries.

A patient with systemic proliferating angioendotheliomatosis involving the female genital organs is reported and discussed. This extremely rare disease has been described in white women ranging in age from 31 to 68. The histology of the ovarian lesion may be mistaken for a reticulum cell sarcoma or dysgerminoma without careful general clinical evaluation by someone who is aware of this uncommon disease.
