The usefulness of single, random measurements of serum FSH and LH in the diagnosis of amenorrhea was investigated. These determinations were performed upon 146 unselected subjects. A wide range of values was found. In primary amenorrhea, normal values were helpful in excluding hypogonadotropic hypogonadism. Low values were not proof of hypogonadotropic hypogonadism, since some subjects could be stimulated by clomiphene to secrete increased amounts of FSH and LH. High values were found in patients with gonadal dysgenesis or unexplained ovarian failure. In secondary amenorrhea, high values were useful in detecting ovarian failure or early menopause. In the remaining subjects, there was no strong correlation between these measurements and the clinical classification of amenorrhea.

Among 22,843 infants delivered at the New York Hospital from January 1, 1966 to June 1, 1971, 2 had congenital goiters. One of these infants had been delivered vaginally while the other was delivered abdominally due to extreme hyperextension of the neck. Both infants were treated with thyroid hormone for two reasons: a) to suppress thyroid stimulating hormone and therefore decrease the size of the goiter and b) to prevent mental retardation due to hypothyroidism. The interesting finding in these cases was the long term history of maternal ingestion of iodine. It is therefore recommended that iodine-containing preparations not be used for extended periods during pregnancy.

Clinical evaluation of the thyroid status of a woman can be erroneous. Thyroid function tests can delineate the relatively hypothyroid woman—a patient whose symptoms, appearance and psychomotor activity resemble those of the true hypothyroid but whose thyroid tests are in the low-normal hypothyroid borderline range. Subjective and objective responses to thyroid medication are dramatic. In this study of 243 women, the state of relative hypothyroidism is described. Recognizing and treating this condition are responsibilities of the obstetrician and gynecologist.

Ninety patients with invasive squamous cell carcinoma of the cervix were seen between 1940–1971. The most common local characteristic associated with vulvar carcinoma was leukoplakia in 21 patients. Despite a significant delay before therapy, two-thirds of the patients had FIGO clinical Stages I and II. Adequate surgical therapy, as defined, in 60 patients resulted in a 66.6% 5-year survival rate; inadequate therapy yielded a 26.6% 5-year survival rate. The absolute 5-year survival rate was 52.5%. Radical excision of the primary lesion must be reemphasized to prevent deaths in the absence of negative lymphadenectomy. Although inguinal or Cloquent's nodes would have been useful indicators in assessing pelvic nodes, the procedure was not employed on a routine selective basis. The size and histology of the primary lesion were related to the incidence of lymph node metastases. Clinical appraisal of inguinal nodes appears to be useful in staging vulvar carcinomas.

Carcinoma in situ of the vulva is, at present, a poorly understood entity and little is known of its etiology and natural history. A case is reported of carcinoma in situ of the vulva in identical twins related to chronic contact of the vulvar skin with arsenical insecticides during farming operations. This report emphasizes the role of hereditary predisposition in the development of cancer and serves to refocus attention on arsenic, a common constituent of insecticides and detergents, as an etiologic agent in carcinomas of the skin.

A child with unilateral ovarian dysgenesis and marked virilization of external genitalia is reported. Cytogenetic studies revealed sex chromatin in cells from a buccal smear and a karyotype of 46/XX in blood and skin cultures. One gonad was a normal appearing ovary with multiple primordial and mature follicles; the other was a gonadal streak with occasional primordial follicles. During the 10 years in which this patient was observed, growth and development were normal, and chronologically, signs of normal puberty appeared. There was no further virilization since birth. Development of breasts and maturation of vaginal mucosa suggested the beginning of ovarian hormonal activity. A review of the literature failed to reveal a similar case. Current concepts of the timing and mechanisms of disappearance of germ cells from the ovary as well as the etiopathogenesis of virilization in gonadal dysgenesis are discussed.

Burkitt's lymphoma was diagnosed in a patient during the third trimester of pregnancy. Treatment was high-dose, intermittent cyclo-phosphamide initially gave a good response which permitted the pregnancy to be carried to term. The infant was normal at birth and had a normal white blood cell count and hematocrit, despite leukopenia in the mother, and he still remains normal at the age of 12 months. Results in this patient and a survey of reported cases in which cyclophosphamide was administered during pregnancy lead to the conclusions that low-dose oral therapy is the least hazardous regimen early in pregnancy while late in pregnancy, large doses administered intravenously may be tolerated without fetal damage.

A simple, rapid and reproducible semiquantitative method for determining the amniotic fluid phospholipid lecithin/sphingomyelin (L/S) ratio was used to study 190 samples from 140 women. L/S ratio and infant birth-weight correlated significantly (r= 0.5). Amniotic fluid L/S ratio correlated significantly with amniotic fluid fat cells (Nile blue stain), bilirubin (ΔLOD400), creatinine and amniotic fluid/maternal blood creatinine ratio. In small infants (<2500 g) with an L/S ratio >2.0, neonatal respiratory problems were not noted. In infants with an L/S ratio <2.0, 6 (33.3%) had respiratory distress and 3 (16.7%) died of respiratory problems. This test is recommended before early delivery to decrease neonatal respiratory problems.

Sixty-five patients with premature rupture of the membranes were studied. Vaginal smears were evaluated with regard to their general aspect, the staining of cellular elements, and the presence of flora, leukocytes, red cells and squames. In two-thirds of the patients special features were observed: dispersal of clumps, weakly staining nuclei and cytoplasm, and a reduction of or the disappearance of the flora. In one-third of the cases, vaginal smears had a normal pregnancy aspect. The cytologic aspects described may be of help in diagnosing certain cases of premature rupture of the membranes.

Patients with eclampsia, preeclampsia, chronic hypertensive vascular disease without superimposed toxemia, idiopathic seizures and in normal labor were studied to determine in what respect serum and cerebral spinal fluid (CSF) from eclamptic patients differed from those of other patients relative to concentrations of glucose, protein, calcium, uric acid and hydrogen ion. All eclamptic patients had elevated CSF protein levels. The increase in protein did not appear to be related to the severity of toxemia nor did the extent of the increase differ greatly from one patient to another. Also CSF from most of the eclamptic patients contained red blood cells, the numbers of which were directly related to the severity of the disease. In addition, serum and CSF from most patients with eclampsia characteristically had an abnormally low pH and elevated levels of uric acid and glucose. These studies should provide a more comprehensive means for diagnosing and assessing the condition of eclamptic patients.
