Four hundred patients with dysplasia have been followed for 1 to 17 years. In 40 patients, the diagnosis was made by smear but could not be confirmed by biopsy or conization. In another 63 patients, diagnosis was made by biopsy but not confirmed by immediate conization. Among these, 10 recurred, 10 progressed to in situ, 3 to microinvasive and 10 to invasive malignancy. And finally 297 patients with cervical dysplasia diagnosed by biopsy were subjected to immediate (within one month) cold conization. By this procedure 66 in situ, 12 microinvasive and 7 invasive carcinomas were diagnosed and treated. The remaining 212 patients were followed without further therapy, and they all had normal findings and negative smears. In time, however, recurrent dysplasia was found in 7.5% of the patients followed and progressed to early carcinoma in another 2.4%. Cold conization is proposed not only as an excellent diagnostic method, but also as an adequate therapeutic measure in patients with dysplasia.

One hundred and eighty-nine patients with molar pregnancy were studied prospectively in order to evaluate the effectiveness of prophylactic chemotherapy in preventing trophoblastic disease. In 73 patients, evacuation was carried out with the adjunctive use of Methotrexate, 0.3 mg/kg/day (20 patients), actinomycin D, 9 μg/kg/day (20 patients), and actinomycin D, 12 μg/kg/day (33 patients) for five days with evacuation generally on the third day. The remaining 116 patients served as controls. Suction curettage was used for evacuation, replacing abdominal hysterotomy in virtually all patients regardless of uterine size. Histopathologic grading of the molar specimen and endometrial curettings proved of some value in distinguishing those patients more likely to develop trophoblastic sequelae when the histologic grading exceeded Group III. A sensitive HCG test was a reliable means for following patients after evacuation. The prophylactic administration of Methotrexate or actinomycin D appeared to eliminate MTD and significantly reduce the incidence of NMTD. Actinomycin D at a dose level of 12 μg/kg/day was found to be the drug of choice. Toxic side effects were minimal The use of Methotrexate was associated with significant hepatotoxicity.

Stromal cell types and hyalinization in the cortex and medulla of the human ovary were evaluated in surgical material from 55 patients. The histologic data were correlated with age, menopausal status, associated clinical conditions and nonovarian gynecologic pathology. Type I and V stromal cells significantly decreased with aging and after the menopause; Type IV cells were significantly more frequent in the presence of obesity. Hyalinization of the ovarian stroma was more frequent as well as marked in old age and after the menopause. The variety of hyalinization that significantly changed was that associated with involution of the lipidic line of ovarian stromal cells. All of these changes took place in the medulla of the organ, the cortex being practically non-responsive. The medullary morphology is compatible with function as an endocrine organ, and the involutive changes described are consistent with those seen in an involuting endocrine organ.

A case of a retroperitoneal mucinous cystadenoma, in the presence of two normal ovaries, stimulated interest in heterotopic ovarian tissue and retroperitoneal tumors of ovarian type. Because an extensive review of the pertinent literature found only isolated references to similar cases and their possible origin, the report of a case and a possible explanation for its genesis seems worthwhile.

Samples of cultivated amniotic fluid obtained at 20 weeks' gestation disclosed the presence of actively growing cells displaying fibroblast-like characteristics. The origin of the cells is discussed: fibroblastic differentiation of fetal ectodermic cells during cultivation versus migration to the amniotic cavity of fibroblasts from the mesodermic layers underlying fetal epithelia.

A 29-year-old patient developed severe Cushing's syndrome shortly after becoming pregnant. The syndrome was characterized by hypokalemia and a marked rise in urinary 17-hydroxycorticosteroids, which could not be suppressed with dexamethasone, 8 mg daily. Clinical and biochemical abnormalities disappeared rapidly after an abortion. Efforts to reproduce the abnormalities afterward with human chorionic gonadotropin and a synthetic estrogen were unsuccessful. This represents the only reported example in which a therapeutic abortion resulted in the remission of Cushing's syndrome.

During a 3-year period, 49 patients underwent hysterotomy for the therapeutic termination of a second-trimester pregnancy. Three patients developed clinical and laboratory evidence of disseminated intravascular coagulation. Common factors underlying the etiology of this serious complication were studied. The coagulopathy developed only among the elderly multipara, and gram-negative sepsis was observed in all 3 patients. Prognosis was related to the severity of disseminated intravascular coagulation rather than to any specific therapeutic modality. Because such elderly patients who present themselves for therapeutic abortion also desire permanant sterilization, a hysterectomy may be the procedure of choice.

Thirty oligo- or amenorrheic women with Stein-Leventhal syndrome were studied by radioimmunoassay of serum gonadotropins and assay of urinary steroids. Serum FSH and LH were determined in 9 of the 30 patients prior to, during and after therapy with clomiphene citrate for 21 or more days. Low, rather tonic serum FSH levels with widely fluctuating serum LH (low, normal or elevated) levels were found in those patients with polycystic ovaries. Administration of clomiphene resulted in an increase in serum FSH, LH or both, with a midcycle surge of both during ovulatory responses. After therapy, a preovulatory peak in urinary estrogens was demonstrated.

It was previously shown in our laboratory that low-dose estrogens stimulated pituitary gonadotropin secretions while high doses inhibited FSH and LH. Based on these observations, 130 patients with irregular ovulatory patterns were given low-dose estrogen on a cyclic basis in hopes of regulating their time of ovulation. Premarin®, 0.1 mg, taken Day 9 to 14 of the cycle was the most effective dose. It regulated ovulation within 3 days in 96.4% of the patients. There were no unexpected premature or delayed ovulations while these patients were under therapy, making the thermosymptomatic method of family planning more effective. It is anticipated that these patients may discard the tedious task of basal body temperature recording while they continue to take low-dose estrogen.
