The pituitary resjmnse to the intravenous administration of 50 jig of synthetic LH-RH was studied in 6 patients with Stein-Lcventhal syndrome and 12 with functional amenorrhea. The results were compared with those obtained from 10 normal menstruating women. LH-RH released comparable amounts of FSH and LH in patients with Slein-Leventhal syndrome and normal eumenorrheic women. Patients with functional amenorrhea showed greater LH baseline levels and greater LH release after LH-RH, however FSH release was not significantly different in the three groups of patients. It is concluded from this study that the LH resjHinse to LH.RH is different between patients with Slein-Leventhal syndrome and functional amenorrhea suggesting that such resjKinse is likely determined by the level of circulating sex steroids and the concentration of endo. genous LH-RH

Because single serum FSH and LH measurements arc of limited use in the clinical management of amenorrhea, the value of multiple determinations was studied. A highly variable and nonspecific pattern of gonadotropin secretions was found, regardless of the clinical classification of amenorrhea. Mean FSH levels did not significantly differ among the clinical types, buc hirsute patients tended to have higher LH values. Increased LH secretion was found in hirsute patients with and without polycystic ovaries, as well as in several subjects with hypothalamic amenorrhea. Patients with low gonadotropin levels did not show a clinical response to domiphene, but responded to HMG as readily as patients with normal gonadotropin levels. In conjunction with basal body temperatures, sequential gonadotropin measurements were useful in documenting ovulation and lutcal phase duration in infertile, menstruating patients

A study of ihe value of electronic fetal monitoring as an intrapartum fetal intensive care method was conducted in a community hospital. The indications for monitoring are listed. The Apgar scores and neonatal mortality figures achieved in 150 cases, 90% of which were “at risk,” arc compared to those of a group of 17,000 unselected cases. The results favor the monitored group. Retrospective analysis of perinatal mortality showed a Pocential 44% saving in infants over 1500 g, if routine monitoring was instituted. No complications ensued as a result of monitoring. Twenty eight cesarean sections were prevented, and 26 were performed earlier because of monitoring. We concluded that electronic fetal monitoring reduced fetal morbidity and mortality in the community hospital

In an attempt to explain poor clinical correlation between amniotic fluid lccitllin/sphingomyclin (L/S) ratio and development of respiratory distress syndrome when the ratio is low, a comparison was made of the phospholipid composition of amniotic fluids in early and late gestation. The results showed an increase in total phospholipid from 2.1 to 5.2 mg/100 ml. an increase of lecithin from 21.0 to 64.8% (0.44 nig to 2.92 mg/100 ml), and a decrease of sphingomyelin from 51.3 to 7.9% (1.07 mg to 0.35 mg/100 ml). Other phospholipids showed minor changes. Amniotic fluid sphingomyelin does not represent extrusion of lung surfactant and some clinical discrepancies might be avoided if, instead of L/S ratio, the percentage of lecithin in total phospholipids (L/PL ratio) is employed

Our fetal maturity formula uses a multiple regression model encompassing the results of fetal biparietal cepbalometry, amniotic Ouid creatinine and estimated fetal weight from 100 high-risk patients. A table was prepared from the maturity equation and tested with data from 54 additional high-risk patients. The predicted gestational ages were compared with the correlated gestationa! ages, as based on menstrual histories and physical and neurologic examinations of newborn infants. The standard deviation of the difference of the predicted versus correlated gestational age was 0.93 weeks. Hence, a successful prediction on the basis of this variability is defined as one in which the predicted gestational age was within 2 weeks of the correlated age. In the prediction group of 54 patients only 2 were more than 2 weeks from the correlated age. This fetal maturity formula, which solves for gestational age, is useful in the clinical management of the high-risk fetus
