During the period from April 1962 through Dec 1969, more than 1400 doses of high-titer anti-D plasma were given to Rh negative mothers of ABO compatible, Rh positive babies, during the first 72 hr postpartum, with no unfavorable reactions. One hundred fifty-four treated patients returned with 176 subsequent Rh positive pregnancies. In 174 pregnancies, sensitization did not occur. Some of the facts and theory underlying this form of treatment are reviewed.

As contributors to the international clinical studies of Rh immune globulin, 3 institutions undertook a double-blind project with data completed for 313 patients, 21 of whom subsequently delivered an Rh positive infant. The safety and merit of this preparation have been established. Proposals are offered for the perplexities that may arise during routine administration; these include the D“ mother or infant, the positive crossmatch procedure, and situations in which more than the usual dosage of Rh immune globulin is possibly needed. Recommendations are made for management throughout pregnancy and labor-delivery of the nonsensitized Rh negative patient.

Others have shown that the oral administration of diethylstilbestrol 25 mg daily for 5 days is an effective postcoital antifertility method. This dose of the estrogen was started in 4 volunteers on the day basal temperature rose. Endometrial carbonic anhydrase, 5 days after the temperature rise, was lowered to an average of 9.8 enzyme units (EU)/g as compared to the average control value of 21.2 EU/g. This decreased carbonic anhydrase activity may represent a barrier to implantation, although other antifertility mechanisms may also be activated by diethylstilbestrol.

Because the primary mechanism of action of intrauterine devices in women is not clearly understood, the development of improved IUD's has been impeded. A great variety of empirically designed devices, largely of linear polyethylene plastic, have been inserted into millions of women during the past decade, often with results inferior to those obtained by Grüfenberg 40 years ago. Therefore, an analysis of factors which could be responsible for variations in IUD efficacy was undertaken. The evidence in hand indicates that a chemotactically mediated, local endometrial response to the device prevents conception. A shield intrauterine device, engineered to take advantage of this principle, has demonstrated a 98.9% efficacy, providing direct experimental support for the surface interaction hypothesis.

A spiral Teflon stent has been developed to prevent adhesion formation after fimbrioplasty. The straight portion of the stent is introduced into the repaired tube for a distance of about 5 cm. The distal portion of the stent, which is formed into a spiral that is maintained with thin-gauge copper wire, is sutured loosely to the fimbria to keep them apart. The end of the stent is brought out of the abdominal cavity through a flank puncture. The prosthesis is readily removed 8–10 weeks, postoperatively, in the office. The stent has been used in 25 patients during the last 18 months. To date, 4 have conceived. Postoperatively, tubal patency, confirmed by hydrotubation and CO, insufflution, has been maintained in all patients.

Of 538 laparoscopies during a 2-year period, 276 were done primarily to evaluate the infertile patient. Analysis of the data obtained indicated that 60% of all patients with unexplained infertility had pelvic abnormalities that were detected for the first time during laparoscopy. The most common finding was minimal endometriosis in one third of all patients with unexplained infertility. Pelvic adhesions were found in 20% of these patients. Lapuroscopy significantly facilitated management of patients with ovulation disorders as well as those with tubal occlusion. Complications were infrequent and not serious. The data obtained were considered sufficiently valuable for us to recommend laparoscopy as an integral part of the infertility investigation.

The gynecologist sees patients with gonadal dysgencsis at a later age than the pediatrician, and his patients usually carry a greater variety of chromosomal aberrations, the most common being 45,X/46,XX mosaicisni. Some patients arc of normal height, have minor or no somatic anomalies, mid even have menstrual cycles, yet they also have some form of cytogenetic abnormality. Clinical findings in 26 gynecologic patients with gonadal dysgenesis were correlated with the results of cytogenetic studies. The only positive karyotypc-phenotype relationship was found between short stature and monosomy for the short arm of the X chromosome. In this series all patients with more than one X chromosome had a positive buccal smear, but the sex chromatin count did not correlate well with differential sex chromosomal findings from peripheral blood, especially in the presence of mosaicisni.

An infant with ambiguous external genitalia and mixed gonadal dysgenesis is presented. Examination of the chromosomes of five tissues indicated the presence of a mosaicism of cells with XO and XY sex chromosome constitutions. We believe that a systematic cytogenctic study in every patient with ambiguous genitalia and sex chromosomal abnormality would aid in possibly finding an explanation for abnormalities of external genitalia and gonads.

A true hermaphrodite with 46,XX/46,XY chromosome complement and with evidence of haptoglobin chimerism suggesting double fertilization is reported. Ten additional case reports of true hermaphroditism with 46,XX/ 46,XY genotypes from the literature are reviewed. The phenotypic features of such patients indicate that embryonic testes, or ovotestes, are less competent than normal testes in müllerian duct suppression and masculinization of external genitalia. The uterus and vagina arc present in a majority of such patients and external genitalia are ambiguous in all of them. The ovary or ovotestis in these patients behaves like a normal ovary as far as development of tubes and breasts is concerned.
