Plasma renin activity, plasma renin substrate, angiotensin I, and plasma angiotensinase activity were measured in 13 women, of whom 5 were normotensive during oral contraceptive therapy (OCT) and 8 became hypertensive during such therapy. In one 21-day cycle of OCT. the mean plasma renin activity increased by two-thirds and the plasma renin substrate more than doubled in both groups of women. The mean angiotensin I increased in both groups— by about two-thirds in the normotensives, and by not quite one-half in the hypertensives. The mean plasma angiotensinase activity increased by one-half among the normotensives; in contrast, it decreased slightly among the hypertensives, only 2 of whom showed increases. Our findings agree with the hypothesis that abnormal inactivation of angiotensin II may have an important influence on the pathogenesis of arterial hypertension complicating oral contraceptive therapy.

Serum testosterone assays were performed on 16 hirsute women and 3 nonhirsute volunteer control patients by obtaining adrenal, ovarian, and peripheral venous blood samples from the femoral vein by a percutaneous catheter approach. Eight demonstrated elevated androgen secretion of adrenal origin (including 1 virilized patient), 5 had combined adrenal and ovarian hypersecretion, and 2 were felt to have elevated serum testosterone of peripheral origin, while 1 hirsute female was found to have no increase in androgen secretion. This method of testing the source of excess androgens is relatively simple, appears to be associated with no morbidity, and does not require lengthy, potentially inaccurate suppression and stimulation studies. Testosterone assays performed by commercial laboratories are available to gynecologists in private practice, circumventing the need of complex research laboratory facilities. Follow-up studies are in progress to determine the accuracy of the functional disorder suggested by this procedure.

The incidence of congenital idiopathic adrenal hypoplasia in a single institution over a period of 13 years was found to be 1 in 12,500 births and 1 in 517 perinatal autopsies. The 11 cases formed a heterogeneous group as there were three different histologic patterns of the adrenal glands. The outstanding clinical feature was the presence of preeclamptic toxemia in 9 of the 11 mothers, 2 of whom had diabetes mellitus. The diagnosis should be considered when maternal estriol excretion is very low and the fetus is alive and radiographically normal. Differential diagnosis from the commoner condition of placental sulfatase deficiency can be made only after birth. Clinical and biochemical diagnosis in life may be difficult.

Female sterilization by laparoscopic technics has many advantages over traditional methods. In this series of 3600 laparoscopic tubal sterilizations, gastrointestinal injury was encountered in 11 patients, 10 of these secondary to electrical burns. The presentation, management, and hospital course of these injuries are presented.

The first 100 patients undergoing laparoscopic tubal sterilization concurrent with therapeutic abortion by suction curettage are reviewed. A detailed description of the combined surgical procedure is given. The surgical time was relatively quick, the postoperative recovery was smooth and rapid (almost 50% of the patients required no pain medication), and, although the follow-up period has been brief, no pregnancies have occurred. Despite all the operations being performed by physicians in various levels of training, only a small number of complications ensued. From this retrospective study, it is concluded that the combination of suction curettage and laparoscopic tubal fulguration can be accomplished with short hospitalization, minimal patient discomfort, imperceptible incisions, and no increase in postoperative morbidity when compared to laparoscopic sterilization of the nonpregnant woman.

A pilot program for outpatient laparoscopic sterilization was begun in 1971. This report describes the development of the program, with emphasis on surgical and anesthetic technics. One hundred and twenty-nine patients participated. Followup of over 90% indicated patient acceptance was high, while the complication rate was low. Although readily adaptable to outpatient performance, laparoscopic sterilization should not become a “minor” procedure since complications, though rare, often require surgical management.

A comparative study of hysterosalpingography (HSG) and laparoscopy in the investigation of infertility is presented. Laparoscopy with permeability testing was performed in 50 patients previously investigated by HSG. Results of these tests showed agreement for tubal patency and prognostic evaluation in 76% of the cases, although neither procedure proved to be free of imperfection. Discussions of both procedures should not be directed to the question of whether laparoscopy or HSG should be performed, but rather which of them should be performed first.

In a prospective, randomized, controlled study of 296 patients undergoing vaginal hysterectomy, the prophylactic use of ampicillin or tetracycline was found to reduce total morbidity (from 62% to 20%), febrile morbidity (32% to 7%), and incidence of severe infections (12.5% to 1.7%). Menopausal status, menstrual cycle, excess blood loss, use of urinary catheter, or presence of associated vaginal repair did not relate to incidence of febrile morbidity. In the antibiotic group, one drug rash occurred, but there were no severe allergic complications.

Thirty-eight patients presenting with septic abortion were treated with intravenous cephalothin, 8 g/day. Eighteen patients treated with other antibiotics served as controls. Based on temperature response and decrease in leukocytosis, cephalothin proved efficacious, but its action was slower than that of the other antibiotics used (in most cases penicillin combined with a broad-spectrum antibiotic). This suggests that penicillin in combination with a broad-spectrum antibiotic is the preferable therapy for patients with septic abortions; cephalothin may be the antibiotic of choice when penicillin allergy is suspected.
