From 1948 to 1967, 113 patients with carcinoma of the cervix associated with pregnancy were seen and treated. Vaginal bleeding was the most common symptom. Thirty percent of the patients had no symptoms. In these patients the prenatal examination and cytologic smear were most helpful. Punch biopsy is an accurate diagnostic tool in both prenatal and postpartum patients, even for Stage I lesions. Gravidas have earlier lesions than their non-pregnant counterparts. During pregnancy, as time from conception increases there is a greater possibility that more advanced lesions will be observed. Age, parity and the trimester in which diagnosis is made have no effect on survival within a given stage. Likewise, the mode of delivery has no effect on maternal or fetal survival. The survival rate is not appreciably different between pregnant and non-pregnant patients within a given stage.

Prolapse of the umbilical cord occurred 516 times among 83,624 deliveries. The incidence was higher in abnormal presentations, particularly transverse lie (7.1%) and breech (6.6%). The gross perinatal mortality was 38.1%; for term infants the incidence was 17.6%. Prolapse of the cord was significantly higher in women over 30 years of age. One hundred forty-seven cesarean sections (28.5%) were performed. The gross perinatal mortality rate was 14.9%; the rate corrected for fetuses over 2500 g and alive on admission was 11.4%. Frequent examinations during labnor help to detect the complication early. When vaginal delivery is not feasible immediately, more liberal use of cesarean section will increase the salvage rate of infants, over 2000 g, with prolapsed cord.

A case report of traumatic laceration of uterine support is presented. This clinical entity, popularly known as the universal joint syndrome or masters' windows, is characterized by the myriad symptoms associated with the chronic pelvic congestion that develops after an obstetric event. The clinical finding is that of an engorged, retroflexed and enlarged uterine corpus that moves freely on the cervix. The broad ligament defect may be seen during laparatomy. A plea for the recognition of this syndrome is made and its surgical treatment is discussed.

A case report of carcinoid tumor metastasizing to the ovaries is given along with a review of the literature on primary and secondary carcinoid tumors of the ovaries. The etiology, occurrence, pathology, spread and prognosis of carcinoid tumor are also discussed.

Amniotic fluid optical density readings of .000 were related to the onset of spontaneous labor. There were 272 amniocenteses performed upon 160 patients, 84 of whom had one or more .000 readings. EDC and .000 Δ optical density had a closer relationship to the onset of labor than either EDC or birth weight alone. A group correlation between .000 density and the time of onset of labor was noted. Three of 5 patients went into labor within 2 weeks, and 3 of 4 patients within 4 weeks of the reading. The rate of error in individual patients indicates that this test alone is not sufficient and should be supplemented by other findings.

Amniotic fluid osmolality was determined on a total of 296 samples from 225 pregnant women. Although a slight decrease in osmolality was observed with advancing gestational age, these measurements have no clinical value in assessing fetal age or welfare. The wide range of values for each disease and gestational age studied negated the usefulness of this test.

Total estrogen concentrations in amniotic fluid were determined in 9 normal and 12 complicated pregnancies. Ammonium sulfate precipitation of the conjugated and free estrogens was employed. Solutions of total estrogens were assayed fluorometrically by the Ittrich procedure. Tracer studies demonstrated that conjugated and free estrogens in amniotic fluid may be isolated in high yields by ammonium sulfate precipitation. Complicated pregnancies exemplified conditions affecting the fetus such as Rh isoimmunization and anencephaly. Lower values were noted in association with severely affected fetuses, but normal and abnormal groups were not completely differentiated.

Heat stable alkaline phosphatase (HSAP) is produced by the human trophoblast and appears to reflect placental function accurately. A study of some 700 normal patients has shown that the pattern of HSAP production occurs in a smooth curvilinear fashion throughout pregnancy, the output being greatly increased during the second half. A study of some 3,000 abnormal pregnancies including preeclampsia, which is discussed here, has shown a significant increase in output of the enzyme by these gravidas. Of particular clinical importance is the fact that abnormal HSAP production precedes clinical evidence of impending fetal disaster by 2–3 weeks. Thus, HSAP estimations are of considerable clinical value in predicting impaired placental function and in determining the opportune time to terminate pregnancy.

The work of Hodgkinson and Green, fortified by Shingleton and others, has brought about an era of high-grade selectivity in procedure never before available to patients undergoing operation for stress incontinence. The ability to select a procedure presupposes a basic understanding of the pathologic processes at work, and it follows that the chosen procedure must be capable of reversing the known pathologic processes, preferably in a progressive stepwise fashion. Of all the procedures designed during the distant or recent past, only the modern Kelly plication and retropubic plication of Ball, together known as the Ball combined procedure, offer true correctional capability with regard to the pathologic anatomy of stress urinary incontinence.

The thirteenth case of malignant mixed müllcrian tumor of the fallopian tube is reported. These tumors occur predominantly in post-menopausal women. Mean age of the patients at the time of operation was 52 years, and mean survival after diagnosis was 7 months. The longest survival was 15 months. Comparison with clinical data on mixed müllerian tumors of the endometrial cavity reveals that for both sites the behavior and fatal outcome of the neoplasm is the same. The histogenesis of mixed müllerian tumors of the fallopian tube is controversial. Available information suggests that the tumor arises in the tubal stroma rather than in foci of endometriosis.
