During the years 1960-1966, 537 patients with FIGO Stage Ib carcinoma or the cervix were treated by combined preopcrative radium and Wcrlhcim hysterectomy with Iymphadcnectomy at the Norwegian Radium Hospital. The overall five-year survival was 88.3%. There was a 2.8% incidence of significant urologic complications. The study concluded that preopcrative radium allowed for a less extensive uretcral dissection and a reduction in the size of the vaginal cuff except in those cases demonstrating residual cancer in the hysterectomy specimen. The finding of positive pelvic lymph nodes was the most significant prognostic factor. The fiveyear survival was 62.9% for those patients with node involvement as compared with 92.9% for those in whom lymphatic extension was not appreciated.

A total of 1410 patients were referred for colposcopic evaluation of the cervix. These included 1206 cases with abnormal cytology and 204 with a grossly suspicious cervical lesion. Diagnostic conization was performed in only 80 patients (5.6%). Two hundred and twentysix cases of severe dysplasia or carcinoma in situ were diagnosed by directed biopsy, while definitive surgical treatment of 181 cases, without conization, revealed no instance of invasive carcinoma. The recognized false-negative rate of directed biopsy in this study (more than 1histologic degree in the surgical specimen than the directed biopsy) was 2 cases among 659 patients (03%). In 160 patients with carcinoma in situ the referring cytology was only “atypical” in 21.2% and the diagnosis was established by colposcopy and a directed biopsy on the initial examination. Eighty-nine pregnant patients with abnormal cytology were referred for colposcopic evaluation and a diagnostic conization was avoided in all. In 25 with severe dysplasia or carcinoma in situ, diagnosed by directed biopsy during pregnancy-, definitive surgical treatment was postponed until after the puerperium, without advancement of the disease.

Punch biopsy before cervical conization and/or hysterectomy was performed in 168 patients during the period January 1, 1968 to December 31, 1971. Diagnoses ranged from dysplasia to invasive carcinoma. When the results of punch biopsy and cervical conization were compared, the diagnostic accuracy of the former technic was 94% and that of the latter 99%. Unsuspected invasive carcinoma occurred after conization in 1% and residual neoplasia in 60%. Reasons punch biopsy failed to reveal the correct diagnosis were: a) inadequate tissue,b) histologic did not correlate with cytologic results, c) neoplastic changes were high in the endocervical canal, d) biopsies were difficult to obtain from patients with atrophtc cervices in their late reproductive years. False-negative results of Papanicolaou-stained smears, in these series, were high (22%), correlating well with the incidences in other series. From these results and a review of the literature, it was concluded that: a) a compromise approach using punch biopsy as an initial screening test followed by conization in selected cases can be supported. The success of this program depends on adhering strictly to the conditions set forth by Holly and by Griffiths et ah If the diagnosis is doubtful, conization is required. Such an approach would assure correct diagnoses and spare many patients unnecessary hospitalization for conization with its subsequent wellknown complications.

Ultrastructural features of the fully developed ciliated endomctrial cpithclia are cilia arising from basal bodies, coated with an outer sheet derived from the cell membrane and connected with the basal bodies by a basal plate. From the basal bodies striated rootlets reach into the cytoplasm. The ciliogencsis occurs intracellularly.Basal bodies arc built by the replication of ccntrioles within the cell. The basal bodies migrate to the cell surface and a vesicle is formed by the infolding of the cell membrane. The basal bodies arrange themselves around the vesicle, and cilia arc built. The cilia externalize when the vesicle opens into the glandular lumen. The function of ciliated cells in the endomelrium is unknown. The morphogenesis of the sparse ciliated cells within the endometrium is apparently stimulated by estrogens.

Management of vulvar atypia with 2,4-Dinitrochlorobcnzc (DNCB) -induced hypersensitivity is described. Treatment was successful in 2 patients, 1 of whom failed to respond to topical 5-fluorouraciI (5FU). Treatment failures were noted in patients who had previously been subjected to multiple surgical excisions and/or to radiation of the vulvar skin lesion. In tile successfully treated patients, no recurrence of disease has been noted in 9 months followup. The value of iinmunochemotherapy in vulvar atypia awaits further evaluation, but the results suggest that in young women with localized disease this mode of therapy is worth trying before resorting to surgery.

Although the incidence of ovarian carcinoma coexistent with pregnancy is only 1:25,000, these tumors represent 39o of all ovarian masses subjected to laparotomy during pregnancy. In general, pregnant patients with ovarian malignancy should receive immediate optimal therapy, regardless of the stage of gestation. The occasional patient with a FIGO Stage IA tumor of the mucinous type or hislologically low grade malignancy, can be treated with unilateral adnexectomy during pregnancy, and reevaluatcd for further therapy postpartum.

Surgery for ectopic pregnancy need not be ablative. Early diagnosis based on the characteristic history, pelvic findings and diagnostic aids will permit surgical intervention in most patients before rupture occurs. Salpingotomy and other operative procedures are described which preserve the patient's reproductive capacity. In this 20-year review of 141 ectopic pregnancies, conservative surgery was performed in 24% during the first decade and in 53% during the last decade. Ectopic pregnancy recurred in 7 patients (all in the last 10*year series), 5 having had initial conservative surgery. Over the 20*year period of conservative surgery for ectopic pregnancy, 30 patients have had subsequent pregnancies; for a total of 32 viable births. Of 40 patients in whom conservative tubal operations were performed, five of the operations were successful; the contralateral tube had either been removed previously or was obstructed.
