A patient with lipoid-cell tumor (adrenal-like cell) of the ovary had elevated 24-hr urinary excretions of testosterone (69 μg) and epitesto-sterone (192 μg). Eighty percent of patients with lipoid virilizing tumors of the ovary (adrenal cell type) experience symptoms for longer than 1 year, and 50% for more than 4 years. The diameter of the tumor usually is not larger than 5 cm. Preoperative diagnosis often is suggested through careful evaluation of clinical data and confirmed by specific androgen fractionations. A diagnostic plan is presented to aid in distinguishing the ovarian causes of virilization from those due to adrenal etiology.

Twenty-six fertile women who had recent coitus were studied for the presence of uterine sperm at the time of vaginal hysterectomy. The presence of spermatozoa in the uterus correlated directly with high sperm density of the cervical mucus approximately 25–41 hr after coitus. When uterine sperm were present, they were associated with a late proliferative phase endometrium. Sperm degradation occurs in the extracellular fluids of the vaginal cavity and endocervical canal. Phagocytosis of spermatozoa by neutrophils and mononuclear cells occurs in both the vaginal and cervical fluids.

The hemagglutination-inhibition antibody test was used to identify rubella-susceptible women admitted for delivery. Prior to vaccination, each vaccinee agreed (signing an informed consent form) to adhere to a reliable method of birth control for at least 2 months. The Cendehill strain of rubella virus vaccine was given to 115 rubella-susceptible women a few days after they had delivered. The vaccine produced an hemagglutination-inhibition antibody response in 98%. The geometric mean titer was 1:60; the range was 1:8 to 1:1024. Unlike the HPV-77 derivatives in adults, the Cendehill strain produced a low incidence of side effects; mild arthralgia was reported in only 4%. Epidemiologic results suggest that natural immunity to rubella is higher in lower socioeconomic groups.

Selective ovarian angiography has been demonstrated to be a useful refinement in defining the type and location of various forms of trophoblastic disease. Of greatest significance is the enhanced ability of the method to differentiate malignant trophoblastic disease from the various benign forms when tumor tissue is inaccessible. Ovarian angiography was performed on 3 patients who had varying forms of trophoblastic disease. Because of the ease with which it is performed, this procedure is recommended in all cases of trophoblastic disease to complete the study of the vasculature of the uterus and pelvis as well as to define the location of obscure foci of disease.

Serum oxytocinase determinations were performed in 30 patients with hydatidiform mole and in 9 with choriocarcinoma. Enzyme activity was significantly lower in those with trophoblastic disease than in women with normal pregnancies of corresponding length. Being a simple, economic, accurate and rapid procedure, serum oxytocinase determination could be used as an aid in differentiating hydatidiform mole from normal single or multiple pregnancy.

An intramural cervical pregnancy which fits all criteria for this diagnosis is described. Some theories of the mechanisms involved in the production of this unusual ectopic pregnancy are presented, and questions are raised as to possible factors in this patient.

A procedure for the early detection of developing primary ovarian neoplasms is needed before a substantial decrease in the number of deaths due to ovarian cancer can be attained. Our prior work indicated that the female mouse was the laboratory model most comparable to the woman, from the standpoint of peritoneal fluid cellular composition which was similarly affected in mice and women by estrogen, the hormones of pregnancy, inflammation, and acute radiation. For 1 year we studied the peritoneal fluid cellular distribution and morphology of peritoneal fluid cells in mice of the C3B6F1 W-series, genotype Wx/WV, all of which developed spontaneous complex tubular adenomas by 7 months of age. The similarity between the histogenesis of this mouse ovarian tumor and the most common ovarian tumors in women was recorded. We could detect the development of primary ovarian neoplasms by a significant change in the cytodifferential cellular patterns of peritoneal fluid. These observations occurred months before morphologic cellular variations appeared in peritoneal fluid, and their value as a possible diagnostic aid for primary ovarian neoplasms in women is stressed.

Heparin therapy was used in 17 patients with septic pelvic thrombophlebitis. It is concluded that medical treatment with antibiotics and heparin is the preferred primary method of management, with ligation of the inferior vena cava and ovarian veins reserved chiefly for patients in whom medical treatment fails.
