Reported cases of the adenoid basal variety of cervical neoplasia are infrequent. However, the fundamental differences in its biologic behavior justify documentation of these tumors. Three cases of invasive disease and two instances of superficial alterations, which are representative of this unusual type of cervical neoplasia, are compared with similar tumors arising in the skin and appendages. Histogenesis and clinicopathologic features of these atypical neoplasms are discussed.

A simple and rapid method to determine urinary estriol has been sought ever since estriol excretion during pregnancy was shown to reflect fetal condition. The two commercially available methods, the Tekit® and Stanbio® kits, were evaluated by comparing the results with those obtained by gas chromatography. Results obtained with the Tekit method correlated well with a correlation coefficient of 0.90 (P<0.001), while that of the Stanbio method was 0.69 (P<0.001). With the Tekit method, the recoveries of3H-estriol and3Hestriol- 16-glucosiduronate as well as the effect of glucose in urine were studied. Since the Tekit method can be carried out by any laboratory, it brings estriol determination within the reach of every obstetrician.

Two case reports of silent uterine perforation by the IUD, TCu-200, are detailed. This relatively new device is currently under study at Bellevue Hospital. Preliminary data collected by the Population Council indicate minimal side effects as well as a low expulsion rate. The addition of copper wire has enhanced its contraceptive effectiveness. No perforations were reported in more than 4,000 initial insertions. The two perforations recorded here occurred when two arms of the device penetrated the cervical canal at the level of internal os. Neither of the patients had any- serious complaints, except that they were disturbed by the “long threads” of the device. Both devices were removed easily without further complications.

The specific stroma of the ovary, particularly its vascularization and involutive changes, has been evaluated in relation to the ovofollicular apparatus and mechanisms of ovulation. Five stromal cell types are recognized which differentiate along either a nonlipidic or a lipidic line. The cellular and vascular features of the ovarian cortex and medulla are very different, reflecting the different functional properties of cortex and medulla. The structural features would appear to play an important role both in follicle migration within the ovary and in the process of ovulation. Various forms of hyalinization of the specific stroma of the ovary are also described. Each variety probably represents involution of different stromal cell types.

A hilar cell tumor of the ovary associated with virilism in a 63-year-old Negro is reported. By light microscopy, this neoplasm was unique due to the large number of Reinke crystals present. Mature Reinke crystals or elementary tubular inclusions were observed either in the cytoplasm or nucleus of neoplastic hilar cells. The ultrastructure of both types of inclusions morphologically resembled that of Reinke crystals observed in Leydig cell tumors of the testis. Elementary tubular inclusions are thought to be precursor units for the development of Reinke crystals.

A clinical and histopathologic study of 59 patients with mesonephroid carcinoma of the ovary is presented. Special attention was paid to whether the tumor was composed predominantly of clear-cell elements or tubular structures, and to the grade of cellular differentiation. Concomitant endometriosis was found in 23.7%, supporting the concept that some histogenetic relationship may exist between mesonephroid carcinoma and endometrioid tumors of the ovary. No major differences in clinical behavior were found among the various histologic subgroups. The most important indication of prognosis was the extent of the tumor. The results for Stage I tumors were almost as good as those in the so-called potentially malignant stages of other histologic types. When the tumor spread beyond the ovaries, however, prognosis was worse than that for other differentiated, malignant, epithelial tumors of the ovary.

In six patients with gonadal dysgenesis, ovarian function was evidenced by menses, which persisted in one patient for as long as seven years after the onset of menarche. Three patients had abnormal sex chromosome karyotypes in peripheral blood. Four subjects, with leukocyte cultures of 45/XO, 46/XX, 46/XX and 46/XX, respectively, had similar karyotypes in skin biopsy material. Those patients with abnormal sex chromosomal complements tended to be somewhat shorter in stature. No correlations could be seen between the sex idiogram and the duration of ovarian activity. The importance of considering gonadal dysgenesis as a differential diagnosis in all patients with secondary amenorrhea, especially when the menstrual life has been brief, is stressed.

The course of a term gravida with acute water intoxication secondary to psychogenic polydipsia is described. The gradual increase in serum sodium from 104 mEq/liter to 128 mEq/liter with the subsequent onset of labor and the delivery of a viable infant are noted. Some aspects of the cause-and-effect of acute water intoxication are discussed.

Inulin was injected intravenously in two groups of gravidas during weeks 38 to 40 of their normal pregnancy. In one group, inulin was injected 24 hours before cesarean section, and in the other, immediately before operation. Samples of amniotic fluid, newborn's urine, and cord blood were then examined for inulin. Inulin injected intravenously into gravidas at term passed to the fetal circulation, the newborn's urine and to the amniotic fluid. The fetal kidneys were found to be capable of clearing and concentrating inulin. In spite of its high molecular weight, inulin passed to the fetal side as an unchanged polymer. The mechanism of passage or transfer of inulin from the maternal circulation to the fetus has not yet been elucidated.

To determine the hazards for infants delivered of mothers receiving diphenylhydantoin during pregnancy, 51 drug-treated pregnancies and 50 control pregnancies were compared. Three congenital anomalies (5.8%) occurred in the drug group and none in the control group; however, this difference was not statistically significant. One drug-group neonate developed a bleeding tendency with an associated abnormal prothrombin time. The literature on the teratogenic effects and the coagulation defects produced in the neonate by anticonvulsant drugs is reviewed.
