It is averred that present-day auscultatory evaluation of fetal heart rate has limited value for the assessment of fetal well-being. A simple-to-operate, convenient fetal monitor and associated methodology have been developed for routine obstetric use. The first year's experience indicates that continuous records of FHR patterns and uterine contractions obtained with the new fetal monitor are of value in patient management. In a monitored group of 245 high-risk patients, there was a decrease of about 75% in primary cesarean-section rate for clinically diagnosed fetal distress, and the incidence of depressed babies was lower than expected.

Spectrophotometric studies of amniotic fluid specimens from 54 Rh-positive pregnancies from the tenth to the twenty-fourth week of pregnancy were carried out. All of the fluids demonstrated a significant absorption peak at 450 mμ. During early normal pregnancy, there is a level of pigment in the amniotic fluid, which later in pregnancy would be considered abnormal. Erythroblastosis fetalis is characterized by the persistance and exaggeration of pigment found in early, normal, nonaffected pregnancies.

A selected series of 83 women in the high-risk pregnancy category were followed by metabolic urinary estriol determinations during their last trimester. Twenty-five were carefully controlled by prescribed bed rest, whenever indicated clinically or by decreased estriol level as determined by serial assays. Absolute bed rest almost always resulted in increased urine volume and urinary estriol output. All of these women were delivered of apparently healthy infants. In comparison, estriol-determination data are presented from 41 women who also delivered viable babies and 17 whose babies died; strict bed rest was not an integral part of their prenatal care. Their estriol output varied and was not predictable.

The potential benefits and pitfalls of the estrogen-creatinine ratio are discussed. Preference is expressed for complete 24-hr urine estrogen determinations as a means of serially assessing fetoplacental function for correlation with the clinical status of the pregnancy under study. However, the estrogen-creatinine ratio does show definite progression throughout pregnancy and there are situations in which an estrogen-creatinine ratio may be of value in lieu of a complete 24-hr collection.

A new resin-sponge uptake method has been devised for the measurement of the serum thyroxine-binding globulin values. Since thyroxine-binding globulin levels reflect serum estradiol concentrations, this method serves as a useful index of fetal-placental viability in patients with threatened abortion. In contrast, it does not appear that thyroxine-binding globulin measurement is useful to determine the viability of the conceptus at term in spite of demonstrated lowered estrogen excretion levels. The latter is probably because the dose response of thyroxine-binding globulin to estrogen administration is not linear and is insensitive to variations in estrogen levels at higher concentrations than are found in term pregnancies. Additionally, this simplified technic for determining thyroxine-binding globulin values may be of use in the clinical evaluation of altered serum thyroxine levels secondary to androgen, estrogen, or anabolic steroid therapy.

Pedometer-recorded physical activity of 34 menstruating women showed three peaks during the menstrual cycle—one at each end and one in midcycle. The latter, coinciding with the time of ovulation, suggests that women have the same type of estrus-induced motor activity characteristic of many other mammals. The two end peaks support the viewpoint that ovulation is not essential to the production of such augmented activity.

A glass electrode was designed for direct in-vivo measurement of endocervical pH. Daily recordings were made throughout the normal menstrual cycle in 3 healthy parous patients. All the readings were between pH 6.5 and 7.5, with an average around pH 7.0. A distinct cyclic pattern was noted with the more alkaline readings during menstruation, at ovulation, and premenstrually.

Intravenous glucose tolerance tests were conducted in 3 groups of women on two different occasions during a menstrual cycle—on Day 14 (estrogen peak) and on Day 24 (progesterone dominance). The first group took no medication, the second group received a sequential-type contraceptive compound, and a third group was given a progestational-type oral contraceptive preparation. Only the second group showed a slightly reduced glucose tolerance in the first test (Day 14); but it was not statistically significant It was concluded that endogenous and exogenous estrogen changes during the menstrual cycle in women had no significant effect on carbohydrate metabolism.

Forty-four women using either Demulen, Deladroxate, or an intrauterine device for contraceptive purposes were studied for urinary tract disorders. Excretory urograms and serum creatinines were performed before contraception was initiated and thereafter at six-cycle intervals. Monthly urine specimens were cultured and examined cytologically. No effect on the urinary tract was noted by X-ray studies and the creatinines all remained normal. Cytologic studies also revealed no abnormalities. The incidence of positive urine cultures was 6.8% initially and 11% during the course of study. Only one woman developed symptoms of infection. A review of the urograms of patients followed in the Family Planning Clinic indicated that dilatation of the upper urinary tract cannot be attributed to anovulatory drugs.
