Findings in a long-term follow-up study of 97 young women with endometrial hyperplasia are presented. In 24 patients (25%), the endometrial hyperplasia was associated with sclero-cystic ovaries consistent with the so-called Stein-Leventhal syndrome. Fertility in all of these patients was low; only 26 (27%) conceived in the years after endometrial hyperplasia was diagnosed, in spite of the desire and efforts to conceive. Only 20 patients (21%) carried their pregnancy to term. In 14 patients (14%), the lesion progressed to adenocarcinoma 1 to 14 years after the initial diagnosis of endometrial hyperplasia. Thirty-seven patients (38%) continued to have menstrual irregularities from 1 to 30 years after the diagnosis. Forty patients (41%) had a hysterectomy by the sixteenth year after the diagnosis. Outlined is a planned, aggressive, but flexible method of management that would promote a high conception rate and less chance for the endometrial hyperplasia to progress to adenocarcinoma.

Plasma FSH levels were determined by radioimmunoassay in 115 pregnant patients. Half of the samples contained FSH levels less than the minimum sensitivity of the assay system. A mean value of 13.4mIU/ml 2nd IRP HMG was obtained. No particular pattern of FSH plasma levels was seen during pregnancy.

Urinary estrone, 17β-estradiol and estriol have been estimated in 10 patients with hydatidiform mole and in 3 others with choriocarcinoma. Urine samples, obtained from patients in a hospital in Mexico, all had very high titers of chorionic gonadotropin. In patients with hydatidiform mole, the urinary concentrations of estrone, 17β-estradiol and estriol varied considerably. They were generally lower than those in women with normal pregnancies of similar gestational length but higher than those in nonpregnant women. In patients with choriocarcinoma, estrogen excretion was within the normal nonpregnant range.

In spite of the apparent value of pelvic arteriography as an adjunct in the initial diagnosis of malignant trophoblastic disease, there appears to be reason for using particular caution in interpreting arteriograms after chemotherapy. A case report demonstrating this point is presented, and the available literature is summarized.

It may be assumed that the proper application of forceps to the fetal head can help preserve the integrity of the cranial structure by assisting in the normal completion of the mechanism of labor. However, the recognized pathologic alterations in normal physiology of the fetus during labor and delivery must be borne in mind. The degree of compression must be minimized so that the balance will be in favor of the amount of stress resulting from forceps use, as compared to the forces at work during spontaneous expulsion.

This study shows that placentascans can be done with a low dose (5–10 μc) of 131I RISA and demonstrates that anyone with a modern scanner can, without the more expensive isotope generators, do accurate placenta localization with a minimal dose of radiation.

Placental growth studies in man and other mammals show that placental mass increases in spite of diminishing rates of cell replication as pregnancy progresses, implying that the weight of the human placenta increases linearly while DNA synthesis is diminishing. The present work utilizes measurement of nuclear DNA synthesis by radioautography. Placental specimens from 44 normal human pregnancies were collected within 1 hour after delivery and autoradiographed by incubation with culture medium and3H thymidine. DNA synthesis was expressed as number of labeled cells/1000 in the mesenchyme and the trophoblast cell populations of the placental villus vs weeks of gestation. The data support the characterization of early placental growth as being hyperplastic, and that of later placental growth as being primarily hypertrophic.

Urinary estriol was estimated before abruption in 5 patients who suffered severe abruptio placentae. Estriol values were normal in 4 patients. The 1 patient with low values had chronic renal disease and associated preeclampsia. None of the infants was small for dates. However, normal urinary estriol values provide no assurance against imminent severe placental abruption.

Primary endometrial stromatosis of the ovary is reported in a 47-year-old woman. The neoplasm was confined to the ovary and arose in an area of ovarian endometriosis. The gross and microscopic characteristics were identical to those of stromatosis originating in the uterus. There was no neoplasm or excessive stromal proliferation in foci of pelvic endometriosis, uterine adenomyosis or uterine endometrium.
