Gonadotropin and steroid hormone excretion in trophoblastic neoplasia and the changes in excretion during treatment of these conditions were studied. The human chorionic gonadotropin (HCG) excretion was high in cases of hydatidiform mole, and was comparatively low in those with choriocarcinoma and chorioadenoma. In all cases with a favorable prognosis, the HCG level fell to the pituitary luteinizing hormone (LH) level with treatment. The urinary follicle-stimulating hormone (FSH) titer in trophoblastic neoplasias was within 20 IU/day and was not influenced by the development of the tumors. Although both choriocarcinomas and chorioadenomas give rise to excretion of large amounts of HCG, pregnanediol and estrogen values were low. A somewhat high value was observed with hydatidiform mole. In patients who died after severe metastasis, an increase of not only HCG but also pregnanediol and estrogen was observed. Except in cases with a large lutein cyst, the 17-ketosteroid (KS) value was within normal limits and the 17-hydroxicorticosteroid (OHCS) value was normal.

A series of adolescent and postadolescent females who had been exposed to stilbestrol prenatally were thoroughly examined and biopsied. At least four nonmalignant lesions not previously noted in such patients occurred with high frequency and were thought to be related to the use of stilbestrol. The lesions were an upper vaginal hood, a strawberry cervix, a cervical portio cock's comb, and an endocervical tumor formation. These lesions can be palpated with one-finger examination. While no relationship of these lesions to adenocarcinoma was demonstrated, the need for further followup of these patients and others similarly exposed is emphasized. Adenocarcinoma was seen in 1 patient out of 15 satisfactorily examined.

A review of the literature reveals no useful means by which reasonable prognosis can be rendered to an infertile patient who has had conservative surgical therapy of endometriosis. Based on a study of 107 infertile patients who underwent conservative operative treatment of endometriosis, a practical classification of endometriosis has been devised to establish the likelihood for subsequent conception. A definite correlation was demonstrated between these factors and the probability of conceiving.

This is the tenth reported case of uterine tube prolapse after abdominal hysterectomy. Hematoma, infection, and certain details of the surgical procedure appear to predispose the tube to prolapse. Differential diagnosis, definitive diagnosis (which requires biopsy), and treatment are discussed.

Thirty-nine of 1850 obstetric and gynecologic patients with endocervical and rectal cultures were suspect forN. gonorrheaby initial culture and test for oxidase positivity. Testing for glucose fermentation revealed that 8 of the 39 patients hadN. gonorrhea, and the remainder hadMima polymorpha, var. oxidans. From an initial suspect group of 2.1% (39 of 1850), an actual infection rate forN. gonorrheawas shown to be only 0.44% (8 of 1850).

A patient with an early abdominal pregnancy associated with an intrauterine device (IUD) in situ is described. Criteria for diagnosing primary peritoneal pregnancy and the relationship between ectopic gestations and in situ IUDs are discussed.

In the original case report of spontaneous regression of Bowenoid vulvar atypia it was suggested that development of the neoplastic changes might represent an unusual sequel to an active infection withHerpesvirus hoministype 2. In the patient presented here, no evidence of such an herpetic infection could be detected. The 2 patients arc however remarkably similar in other parameters including: age, association of the lesions with pregnancy, and persistence of the neoplastic papules for 5 to 6 months prior to spontaneous regression.

Metastatic and secondary tumors of the vulva, though uncommon, were the third largest group of malignant tumors of the vulva at Maternity and Barnes Hospitals. Epidermoid carcinoma of the cervix was the most frequent primary site (46%), followed by the endometrium, kidney, and urethra. Both the primary tumor and the vulvar metastases were diagnosed synchronously in 6 patients (27%). Most patients who subsequently developed vulvar metastases had signs of advanced primary tumor when initially diagnosed. Metastatic adenocarcinoma tended to invade the overlying squamous epithelium, whereas epidermoid carcinoma did not. The frequent occurrence of vascular involvement by metastatic tumor tended to incriminate this as the mode of spread to the vulva. Only 2 patients were alive, 1 with recurrent tumor; the others were dead of tumor usually within 1 year of the diagnosis of a vulvar metastasis.

Reports have appeared in the literature regarding hypercalcemia associated with ectopic hormone production by malignant tumor, though rarely has this condition been reported with cancer of the vulva. This case report deals with a carcinoma of the vulva producing hypercalcemia which was corrected by the surgical excision of the tumor.

Primarily because of the rarity of cancer of the uterine tube, optimal primary therapy is still not well defined, and there is little information available regarding the efficacy of chemotherapeutic agents in recurrent or disseminated disease. The available literature on this subject is reviewed. In addition a patient with complete objective response maintained over 13 months since the institution of Alkeran®therapy for disseminated tubal cancer is presented.
