In a controlled study, patients were matched according to age, ethnic background, age at first coitus, age at first pregnancy and socio-economic status. Patients with cervical carcinoma had not used oral contraceptives to a greater extent than did control patients. This study is being continued and longer use is being investigated.

Thirteen patients with symptoms of secondary amenorrhea and galactorrhea and 1 with galactorrhea alone have been studied. These symptoms developed in 5 after they discontinued taking oral contraceptives, in 4 after delivery; no apparent cause for the symptoms could be found in 2 patients, a pituitary tumor was diagnosed in 1, cystic mastitis in 1 and in 1 the features developed during treatment with phenothiazines. The possibility that patients could become pregnant either spontaneously or after treatment with clomiphene or pituitary gonadotrophin was most favorable in those who developed their symptoms postpartum. Specific efforts should be made to exclude a pituitary tumor as a cause of the symptoms.

Plasma progesterone levels measured at weekly intervals in 65 women who were being treated with clomiphene or gonadotrophins for anovulatory infertility, proved useful for detecting ovulation in infertile women and for confirming the onset of pregnancy. After treatment with clomiphene mean plasma progesterone levels during ovulatory cycles were higher (10 to 50 mμmg/ml) than those present during spontaneous ovulation (10 to 20 mμg/ml). Patients were distributed into three groups according to frequency of menstruation before treatment. Evidence of ovulation after treatment with clomiphene was observed in patients from each group but occurred more frequently in patients with irregular cycles (27 of 28) than in those with oligomenorrhea (39 of 48) or amenorrhea (5 of 16). Plasma progesterone was measured by a competitive protein binding technic after purification by thin-layer chromatography.

Total and heat-stable serum alkaline phosphatase (heated at 65°C for 30 minutes) from normal and abnormal pregnancies were determined serially using a colorimetric method. In 71 normal pregnancies, enzyme activity increased gradually as gestation progressed. In 61 patients with preeclampsia and in patients with hypertension superimposed on preeclampsia, total and heat-stable enzyme were increased significantly over normal levels. This increase anticeded clinical signs. Enzyme activity was also increased in 19 patients with threatened abortion, and this increase was more significant in those with a previous history of threatened abortion. In prediabetic and diabetic patients, the reduction in enzyme levels varied, but a significant decrease from normal levels was noticed near term. It is concluded that changes in enzyme levels could be a manifestation of functional disturbances in the placenta, but before this test can be evaluated for routine use, further studies are required.

Modifications have been incorporated in the method of Gluck and others for determining the lecithin–sphingomyelin ratio (L/S) in amniotic fluid. After thin-layer chromatography in chloroform:methanol:water, 12:5:1, the phospholipids are visualized by phosphomolybdic acid reagent. The untoward effects of storage, at room temperature, on the stability of L/S and the effects of amniotic fluid contamination with meconium or plasma on the L/S ratio have been investigated. In the followup of 41 pregnancies, low lecithin–sphingomyelin ratios, < 1.0, were confirmed before week 29, correctly predicting the development of respiratory distress syndrome in the premature newborn. In pregnancy of unknown duration or near term, low L/S values oblige one to maintain pregnancy until repeated sampling confirms fetal lung maturity.

Ethanol, administered intravenously to the mother at the time of delivery, traverses the placenta and, within 1 minute, is present in the bloodstream of the newborn. In 22 of 23 patients, serum ethanol levels in the umbilical vein and artery were consistently lower than or equal to those in maternal venous blood drawn simultaneously. After predelivery infusion, postpartum levels of maternal venous ethanol fell in a linear (zero) order or kinetics, with a half-life measured in 1 to 4 hours. The infants' Apgar scores or clinical course in the nursery were not affected adversely when ethanol was administered, in the dosage reported, either before or at the time of delivery. The induction of fetal hepatic as well as placental microsomal enzymes from the maternal ingestion of drugs during pregnancy is discussed.

Histochemical and electron microscopic study of the choriodecidual junction of the human placenta suggests selective deposition of both fibrinoid and collagen at that site. These substances are consistently located in intimate association with decidual cells, which may be responsible for their deposition. The factors at the choriodecidual junction which could stimulate a specific decidual cell secretion are reviewed.

Fetal-maternal microtransfusion complicating transabdominal amniocentesis has been studied in 28 patients who underwent a total of 39 amniocenteses. The numbers of fetal cells in maternal venous blood were counted before and after each tap and the placenta was localized by the B-scan ultrasound technic; these data were not revealed until the patient had delivered. Although significant numbers of fetal cells were found before the first amniocentesis in 5 patients, and fetal cells increased after an amniocentesis that could not have traversed the posterior placenta of 1 patient, 50% of those women with an anterior placenta that was penetrated by the needle had a remarkable increase in fetal erythrocytes. The threat of a significant microtransfusion in an Rh-negative unsensitized woman warrants efforts to localize and thereby avoid the placenta when amniocentesis is indicated.

Three successful pregnancies in 2 patients with proliferative diabetic retinopathy are reported. It is suggested that despite the relatively high incidence of fetal wastage in women with this complication, as reported in the available literature, patients should be managed individually rather than recommend the pregnancy be terminated as a routine measure.
