Patients with dementia often present with challenging behavioural disorders. Paranoid delusions, hallucinations and agitated and aggressive behaviour are common and difficult to treat. The currently available drugs to treat such disorders, especially neuroleptic agents, result in a high frequency of complications including extrapyramidal syndromes. Risperidone, a new neuroleptic, is now available for the treatment of schizophrenia in younger patients. This study describes a series of eleven elderly patients with dementia who were successfully treated with risperidone for the control of agitated and aggressive behaviour. Controlled trials over a longer period need to be conducted.

Respite is effective in alleviating the stress caused by caring for a person with dementia. A questionnaire survey examined the factors associated with the use of, and unmet need for, respite. In the past year, one third of carers had used respite monthly or more; 20% indicated an unmet need for respite at least monthly. Frequent users were likely to be female, providing care for a more elderly person, and obtaining respite from family and friends rather than from formal sources, Unmet need was associated with depression, restricted social activities, family illness, and negative perceptions of formal respite services. Flexibility may be the key element in enabling formal respite services to reduce unmet need.

Various features of the squalor syndrome are described with the aid of scanty existing literature. Flaws in drawing inferences from this limited literature are discussed. It is suggested that this syndrome occurs in all age groups (not just the elderly) with a heterogenous range of psychiatric diagnosis.

A mailed survey of 28 residential facilities for ambulant people with dementia questioning the types of labelling of residents' toilets yielded 24 replies: 42% (10) had no labelling, “Toilet” was used in 50% and “Male” or “Female” was used in 8%. The survey was followed by asking elderly people in an acute medical hospital ward and in an aged care residential complex their preferences for toilet word and symbol labelling There were 21 elderly people with normal cognition, 11 people with mild dementia and 16 people with moderate dementia. The international symbol was preferred by people with normal cognition or mild dementia but a symbol more representative of a toilet was preferred by people with moderate dementia (p<0.05). “Ladies” or “Gents” was the word most preferred by people with normal cognition. “Toilet” was the word preferred by people with moderate dementia (p<0.05). A reduction in disability is not the normal expectation for a resident of a nursing home. In fact, it is usual to expect a decline in ability as the resident's disease progresses. This paper will outline a program that restructured care practices and is now demonstrating improvements in levels of confusion and disability.

The majority of programs to address elder abuse centre on the plight of the victim. In contrast, the Elder Abuse Perpetrator Program deals directly with the abuser. Based on the principle of confrontation and utilising cognitive‐behavioural techniques, the program applies to those cases which have not been resolved through supportive educative measures or by abuser self‐resolution. This paper briefly outlines the program and describes some of the valuable lessons learned since its introduction. Medical and diagnostic services for people aged 65 and over comprise a substantial proportion of all services provided by the Health Insurance Commission under the Medicare Benefits Schedule. To examine the utilisation of these services by people in this age group, we analysed de‐identified data on a cohort of people who claimed for at least one such service in NSW in 1991. The total number of services claimed for by this cohort of 7,724 people was 144,658. The median number of services received by individuals during the year was 13.5 per person for the cohort (or 9.9 for the total group). Of the total services reimbursed, 64.7% were for doctors' consultations, 24.8% were for diagnostic services and 10.6% were for other services, predominantly operations. The most common claims were for visits to a general practitioner for a standard consultation. Service utilisation differed between the age and sex categories of the patients in the study sample. Utilisation tended to increase with age and was more common among women than men for several of the service groups considered. We report the results of these analyses and discuss the potential benefits and disadvantages of using claims data to evaluate utilisation of medical and diagnostic services, and current clinical practice
