摘要:
Since the early days of transplantation, corticosteroids have been the mainstay of immunosuppressive regimens following transplantation. However, the use of corticosteroids has been associated with numerous side effects, including bone demineralisation, diabetes mellitus and hyperlipidaemia. In recent years, tacrolimus [Prograf] has been outperforming cyclosporin as the first choice of immunosuppressant in kidney, heart and liver transplantation. Studies presented recently at the 11th Congress of the European Society for Organ Transplantation (ESOT) [Venice, Italy; September 2003] suggest that treatment with tacrolimus enables the long-term management of transplant recipients without the need for corticosteroid therapy, and can also be cost effective compared with cyclosporin microemulsion.1,2

The results of several open-label clinical studies presented at the 43rd Interscience Conference of Antimicrobial Agents and Chemotherapy (ICAAC) [Chicago, US; September 2003] showed that a new antifungal agent, posaconazole [Noxafil] has promising activity against a number of recalcitrant infections. Many of the patients involved in these studies had haematological malignancies, chronic granulomatous disease or infections of the central nervous system, or had received solid organ transplants; fungal infections in such patients are difficult to treat and often have a poor prognosis. In most cases patients had failed, or were unable to tolerate, their previous antifungal therapy. Pharmacokinetic studies presented at the meeting showed that the bioavailability of posaconazole was not affected by gender or ethnicity, and that the effects of age were not clinically significant. Another study showed that a high-fat nutritional supplement, typical of those often given to patients with fungal infections, increased the absorption of posaconazole.

Montelukast [Singulair], an orally active leukotriene D4antagonist, is effective in preventing asthma attacks triggered by upper respiratory tract infections in children, according to results from an international study presented at the 13th Annual Congress of the European Respiratory Society [Vienna, Austria; September− October 2003]. This study, which involved > 500 children aged 2−5 years, showed that treatment with montelukast reduced the incidence of asthma attacks and prolonged the time to patients' first asthma attack, compared with placebo."This is the first time a treatment option for asthma has been proven to be effective at reducing childhood asthma attacks brought on by the common cold,"said the study's lead investigator, Professor Hans Bisgaard from Copenhagen University Hospital, Denmark.
