Certain aspects of the phenomena considered under the concept of projective identification, notably those concerned with the relationship between intrapsychic and interpersonal phenomena and with indirect forms of communication and influence, are discussed in relation to an alternative theoretical framework. It is argued that these clinically important phenomena are best understood as particular forms of the more general processes involved in interpersonal relating.

The origins and resistance to change of neurotic procedures are considered with particular reference to the nature and role of consciousness. It is argued that the traditional opposition between conscious and unconscious systems provides an unsatisfactory model. The crucial role of language in the formation of human self‐consciousness is emphasized. The restricted procedural repertoire of neurotic subjects, and their deficient self‐consciousness, can be attributed to a number of factors. This paper describes the successful treatment of an unusual case of post‐traumatic stress disorder (PTSD). The case was unusual in that the patient, a 38‐year‐old woman, had neither experienced nor witnessed the traumatic event, but seemed to have experienced PTSD ‘once removed’ from the incident (the murder of her colleague). The four treatment sessions focused on traumatic events in the patient's childhood which had produced no identified symptoms, but which had become intertwined with the adult trauma to provoke the PTSD

This study examined the ways in which five mental health professionals talked about paranoia. A discourse analysis (Potter&Wetherell, 1987) suggested that interviewees positioned themselves, and were themselves positioned, by a number of discourses, or systematic ways of talking, in relation to this topic. These discourses appeared to serve a number of functions including the maintenance of professional legitimacy. Interviewees used a number of rhetorical strategies (including exploiting the range and ambiguity of diagnostic definitions, criteria and theories) in order to meet challenges, for example that paranoid beliefs might be true.

The selection of a case of demonstrated change using a quantitative criterion preceded an exploration of possible change mechanisms in therapy. Between‐session variations in the therapeutic relationship revealed by a quantitative measure prompted an intensive qualitative investigation of in‐session events. Within these events, the study focused on challenges to the therapeutic relationship and their subsequent resolution. Understanding the resolution of such challenges to the relationship was guided by a rational model derived from the treatment rationale. This paper presents preliminary data on the psychometric and clinical properties of the Inventory of Interpersonal Problems (IIP: Horowitz, Rosenberg, Baer, Ureno&Villasenor, 1988) obtained from a UK sample of clients presenting with depression/anxiety for individual psychotherapy as part of two concurrent outcome studies: the Second Sheffield Psychotherapy Project (SPP2) and the Medical Research Council/NHS Collaborative Psychotherapy Project (CPP). The factor structure of the IIP is reported on a sample of 250 clients and eight factor‐analytically derived scales are presented. Data on the reliability of the IIP together with its relationship to the Symptom Checklist‐90R (SCL‐90R: Derogatis, 1983) are investigated in a subsample of 143 clients. In addition, data are presented on the clinical relevance of the IIP using case material. The structure of the IIP was further analysed using ipsatized scores.
