To elucidate the role of intercellular adhesion molecules (ICAMs), which has not been well understood in pancreas, we investigated the localization and expression of ICAM-1 by immunohistochemistry and in situ hybridization (ISH) in pancreatic adenocarcinoma and in normal pancreas. The localizations of ICAM-2 and ICAM-3 were also investigated by immunohistochemistry. In normal pancreas, acinar cells, duct epithelial cells, and Langerhans islet cells failed to stain with anti-ICAM-1, anti-ICAM-2, and anti-ICAM-3 antibodies. These cells showed no expression of ICAM-1 mRNA. On the other hand, various percentages of carcinoma cells were stained with anti-ICAM-1 antibody, while no carcinoma cells were stained with anti-ICAM-2 and anti-ICAM-3 antibodies. ICAM-1 mRNA expression was also observed in carcinoma cells, and ICAM-1 mRNA expression was associated with localization of the ICAM-1 protein. These results suggest that ICAM-1 expression is up-regulated in pancreatic adenocarcinoma cells and that ICAM-1 is involved in malignant processes in pancreas.

The treatment recommendations for intracystic hemorrhage in pancreatic pseudocysts are various. We have used a defined treatment protocol in these difficult cases. The experiences gained are reported here. Patients with clinical signs of ongoing bleeding and with hemorrhagic pancreatic pseudocyst in ultrasonography and computed tomography (CT) were studied with urgent angiography. Patients showing a pseudoaneurysm were treated with immediate embolization and delayed elective surgery whenever candidates for surgery. During a 5-year period 10 patients were treated according to the protocol. Pseudoaneurysm was demonstrated and subsequent embolization performed in six. Angiography was negative in four patients. During the study period two additional patients were treated conservatively without angiography because the bleeding had stopped 2 weeks prior to the referral to our institution. Two patients were electively operated on and the remaining 10 patients were treated conservatively. Fever and elevated transaminases developed in one of the six patients after the embolotherapy. One of the four embolized patients who were not operated on developed pseudocyst infection 4 months after the embolization. One of the 10 conservatively treated patients died (1/10 = 10%; total mortality, 1/12 = 8%) during the follow-up for infection complications of necrotizing pancreatitis 2 months after the initial bleeding. Another conservatively treated patient with negative angiography had recurrent bleeding during the follow-up but could not be operated on due to severe liver cirrhosis. In the remaining eight patients pseudocysts resolved during the 3-month to 3.5-year follow-up as confirmed by CT. The low rates of mortality and rebleeding support a fairly conservative approach for hemorrhagic pancreatic pseudocysts.

To clarify whether β-cell function deteriorates with increasing albuminuria and duration of diabetes, we investigated the insulin and c-peptide response to oral glucose tolerance test in 47 healthy subjects and 75 normoalbuminuric (group 1), 30 microalbuminuric (group 2), and 35 macroalbuminuric (group 3) Type 2 diabetes mellitus patients. The total area under the insulin curve (AUCI) values of groups 1, 2, and 3 were 245 ± 16, 193 ± 17, and 88 ± 8 pmol/L · h, respectively, significantly lower than that (624 ± 34 pmol/L · h) of the healthy group. The mean total area under the c-peptide curve (AUCC-P) values of groups 1, 2, and 3, respectively, were 2.50 ± 0.11, 2.05 ± 0.15, and 1.73 ± 0.07 nmol/L · h, respectively, significantly lower than that (5.47 ± 0.19 nmol/L · h) of the healthy controls. The mean AUCI and AUCC-P values of group 3 were significantly reduced compared to those of group 1. The mean AUCI and AUCC-P values of patients with 0–5, 5–10, 10–15, and >15 years' duration were significantly decreased compared to those of healthy subjects. The mean AUCI and AUCC-P values of patients with >15 years' duration were significantly lower than those of patients with 0–5 years' duration. The mean hemoglobin Ale values of the three diabetic groups were not significantly different. These data suggest that β-cell function deterioration was associated with increasing albuminuria and diabetic duration in Type 2 diabetic patients.

The availability of highly purified human islets has increased with the progressive improvement of isolation methods. This has provided opportunities to perform various in vitro studies on human islets. However, when islets are maintained in culture, the overgrowth of fibroblasts results in a reduced islet purity and often has an adverse effect on islet function. To reduce fibroblast growth and to maintain normal islet function, we have investigated a new three-dimensional culture technique using a noncoated transparent Biopore membrane insert (Millicell CM, Millipore). Islets were isolated from seven human pancreata and cultured for 2 months using this membrane insert. At various time intervals, the functional viability of is lets was assessed by measurements of insulin released into the culture medium, static incubation assays of basal and stimulated insulin release, islet insulin contents, and insulin biosynthesis. Results were compared to those of islets cultured in hydrophobic plastic petri dishes, our standard procedure. We found that the non-coated membrane does not allow islet attachment to the surface and prevents fibroblast growth, so that islets maintain a three-dimensional structure and remain in a free-floating form. Islets cultured in a membrane insert showed a function similar to or better than that of islets cultured in plastic petri dishes.
