摘要:
Loss of glucose homeostasis carries the risk of emergencies at opposite extremes of dysregulation. Hypoglycemic crises arise from high exogenous insulin levels and counterregulatory deficiencies. Conversely, hyperglycemic crises arise from insulin depletion and heightened counterregulation. Emergency treatment aims to identify and reverse the imbalance.

Reserved for patients who fail to respond to NSAIDs or who have severe or rapidly progressive disease, these pharmacologically diverse drugs have no common mode of action. Half of rheumatoid arthritis patients will ultimately need such therapy, and for them, finding a drug that is effective and can be tolerated may in some cases require trials of four or more drugs.

Thanks to the medical revolutions of the past half century, we have attained a remarkable understanding of health and disease. And by virtue of concurrent social revolutions, significant (although imperfect) progress has been made toward more equitable distribution of the fruits of this understanding. Future generations will undoubtedly be intensely interested in the men and women whose work produced these revolutions. The ability to record their thought processes through the techniques of“oral history”can provide invaluable insights. It is in this belief that Hospital Practice is reproducing interviews conducted by cardiologist Allen B. Weisse, Professor of Medicine at the University of Medicine and Dentistry of New Jersey-New Jersey Medical School. They were originally published in 1984 asConversations in Medicine: The Story of Twentieth-Century American Medicine in the Words of Those Who Created It(New York University Press). Following is the May 1981 interview with obstetrician-gynecologist Lena F. Edwards.

The operation of this recently recognized cotransport unit, which tightly couples the translocation of sodium, potassium, and chloride across cell membranes of the thick ascending limb of Henle's loop, is described, along with the pathologic derangements of renal function that give these physiologic phenomena their clinical correlation.
