摘要:

The discovery that normal cells contain proto-oncogenes—genes that are analogous to known viral oncogenes—may provide a shortcut not only to understanding some of the pathophysiologic mechanisms that must be involved in carcinogenesis but also to dissecting the processes of normal cell growth and of the evolutionarily developed controls on such growth.

It has long been recognized that these drugs may be decisive in allowing patients to function. Now, with recently developed strategies, reasonably safe treatment is practical. Guidelines for clinical application of these strategies are offered.

Whether left ventricular hypertrophy is a useful compensatory process rather than the first step in heart failure is under debate. Early experimental data indicate that events during hypertrophy indeed hasten the progession of failure. Since it is often possible to reverse hypertrophy with some forms of antihypertensive treatment, it makes sense to try to do so.

Of the six typeable strains of this organism, type b is the most virulent accounting for 95% of serious infections in children. Unexpectedly, in adults, nearly two thirds of isolates from blood and CSF were found to be nontypeable. The spectrum of clinical disease associated with type b and nontypeable strains is discussed. Effective immunization of infants is now in the planning stages.

The trend in clinical research today is toward increasing reliance on quantitative epidemiologic methods. To evaluate the validity of studies based on epidemiologic principles and their applicability to clinical practice, a working knowledge of the most commonly used observational and experimental study designs is useful. Characteristics of three basic study designs are discussed.
