In this first of a new series of articles on acute and chronic pancreatitis, a rational approach to the management of acute disease is outlined. Because it is often difficult to judge its severity from the initial symptoms, the ability to identify patients who are at risk of major complications and death is critical. Five risk factors can be identified at the time of admission.

An ever-expanding capability of examining genetic material with DNA probes and restriction endonucleases has opened up a new approach to old clinical problems. Attention is focused on genetic disorders involving growth hormone and on implications for the diagnosis of other conditions in which the genetic foundations are less accessible to examination.

Demonstrations that metastatic cancer can respond to or be cured by drugs have led to increased interest in the interaction of drugs and malignant cells. That research has suggested ways in which cancer treatment may be improved—most significantly, by combination chemotherapy and combined-modality treatment. Other strategies are being assessed.

In Part II of the discussion of PMN function, the focus is on ways in which the complex biochemical processes that modulate cell motility and oxygen radical formation can go wrong. Derangements of these functions—hyperactive as well as hypoactive phagocytosis—lead to clinical disorders that predispose the host to infection and inflammatory reactions.
