摘要:

The“second messenger”function of the cyclic nucleotides, CA MP and CG MP, through which external stimuli arriving at the cell membrane are made internally effective, is thought to depend on their ability to activate a class of enzymes known as protein kinases. These then set in motion a cascade of biochemical processes that modulate and enormously amplify the metabolic activities carried on within the cell.

The problem was to narrow the gap between the training provided during residency and the requirements of the practicing pediatrician; the first step came in response to needs obvious in the community surrounding the university hospital. In a few short years the Maryland residency has come to offer training in some 85% of the experiences a pediatrician is likely to encounter after residency—including those of private practice.

Caries results from colonization of teeth by certain streptococci, followed by their utilization of dietary sugars to produce a decay-inducing“sponge of acid”on tooth surfaces. This etiology suggests there are at least three ways to counter caries: eliminate the bacteria, change the diet, strengthen the tooth surface against the acid. Fluoridation accomplishes the latter and has the merit of not requiring individual action.

How best can quality of patient care be assured? No topic on the contemporary medical scene eclipses this one in terms of professional interest. Moreover, legislative as well as professional initiatives are already well under way. AccordinglyHospital Practicehas assigned one of its senior editors to investigate the subject, describe the methods by which quality of care is now being monitored, and relate how the information gained is being used to improve patient care, in continuing education, and to change institutional practices. The editors invite readers not only to comment on these reports but to propose subjects for coverage on the basis of the special facets or issues they consider particularly significant.
