摘要:
When clinical studies of potential gallstone-dissolving agents began, it was thought that all the natural bile components might be suitable and that the goal of pharmacologic dissolution might be achieved through expansion of the bile acid pool. But the experience described here showed that only chenodeoxycholic acid dissolved stones, that it dissolved only cholesterol gallstones, and that it acts by rendering bile unsaturated in cholesterol.

Some 900 patients selected by rigorous criteria will be enrolled at 10 treatment centers widely dispersed around the country when the second phase of the national double-blind cooperative study to evaluate chenodeoxycholic acid as a dissolving agent for cholesterol gallstones gets under way next year. Studies on the mode of action of amphotericin B, nystatin, and related antifungal antibiotics eventually led to investigations on the mechanism by which the complement system damages cell membranes. Animal studies are described that have demonstrated a centroneurogenic rather than hemodynamic etiology for this syndrome. In the presence of profound cerebral hypoxemia, an inappropriate signal from the brain heightens resistance in the pulmonary venule, producing a tourniquet action that leads to congestion, hemorrhage, and atelectasis. Denervating the lung is protective, a finding that suggests new avenues to treatment.
