In recent years, research has expanded the list of the contractile proteins to at least four: two, actin and myosin, whose interaction is primary, and two, tropomyosin and the troponin complex, whose action is modulatory, serving to inhibit the primary actomyosin interaction. As the contractile proteins have become better understood, so too has the role of calcium ion, which can now be construed as the inactivator of the inhibitor molecules.

At least partial correction of many recessively inherited metabolic disorders is now possible, as a catalogue of treatable diseases in this category makes clear. But while therapeutic nihilism need no longer prevail with regard to such conditions, prevention is still to be preferred—and remains the only present approach to the important chromosomal disorders and to practically all dominantly inherited diseases.

The ionization and penetration characteristics of the proton beam permit an ablative dose of radiation to be delivered to the pituitary with minimal effects on other intracranial structures. This approach to hypophysectomy is especially appropriate in conditions like acromegaly, where delay in achieving hypopituitarism is not critical (as in terminal breast cancer). The steps in the procedure are outlined.

When chronic pancreatitis is accompanied by the presence of a mass in the pancreatic region, treatment must be surgical, but the choice of procedure is not always obvious; if the mass is a cyst, simple internal drainage is to be preferred. While the final decision is made at the table, ultrasonic scanning, as in the case presented, can be of great assistance in anticipating the operative findings.

With the availability of clomiphene citrate and human gonadotropins, it is now possible to induce ovulation in the majority of women whose infertility results from failure to ovulate; accordingly, it is extremely important to avoid doing so unless the patient wishes to become pregnant. The physiologic basis for the effectiveness of these agents is detailed, as are the situations in which their use is inappropriate.
