Vastly improved knowledge of the role of IgE in mediating immediate hypersensitivity reactions has provided a scientific underpinning for traditional approaches to allergies like ragweed hay fever and helped explain the effectiveness of long-utilized drugs. It has already led to improvements in diagnostic techniques and is opening the way to the development of better and more specific therapy.

Recent large-scale studies using radioactive tracers to identify deep vein thrombi show that pulmonary embolism is now largely preventable in high-risk hospitalized patients through prophylaxis with low-dose heparin or a pulsating boot. In outpatients with leg pain, the problem of separating the 20% who have a thrombus from the 80% who do not can often be resolved by phlebography performed on an ambulatory basis.

Although endocrinopathies have long been recognized as a source of psychiatric and behavioral symptoms, it is still not easy to pinpoint the relationship between a neuroendocrine dysfunction and the individual's response. Understanding has begun to improve as we have learned that some hormones act directly on specific brain target areas, others act indirectly, and still others have different effects at different ages.

Blockade of the renin-angiotensin system by the angiotensin analogue saralasin may have both diagnostic and therapeutic applications. Response to this investigational drug appears to provide a noninvasive means of identifying patients with renovascular and some other“angiotensinogenic”hypertensions. It is also seen as potentially useful in the management of hypertensive crises or states exacerbated by angiotensin excess.

With legal abortions now numbering close to a million a year, it has become apparent that special units, separate from other OB-GYN hospital facilities, are needed for maximum efficiency and safety. Safer, more efficient methods are also urgently required, especially for use during the second trimester. Studies are described that suggest improvements in intra-amniotic infusion techniques may be close at hand.
