摘要:

Since 1940, radioisotopes—originally and especially radioiodine and more recently other isotopes as well—have been the cornerstone of studies seeking to elucidate thyroid physiology and pathophysiology. The emphasis here is on the current applications of nuclear procedures to the assay of thyroid function, the imaging of normal and abnormal structure, and the treatment of thyroid disease.

Electrocardiography is invaluable in detection and characterization of arrhythmias. But it defines only one of many parameters affecting decisions on whether to employ antiarrhythmic drugs and which ones to use. Other factors include the differences between immediate postinfarction and ambulatory patients and the specific physiologic events underlying the development of an arrhythmia.

The rewards of an in-office laboratory are many: greater selectivity in testing, lower cost to the patient, better care and follow-up. With the many prepared reagents, diagnostic kits, culture materials, and disposable equipment now available, the space and staff requirements of a laboratory in which a wide variety of basic tests can be performed are minimal. Details are given on the organization of such a laboratory.

When obesity is truly massive, is intractable and of long standing, and is not associated with abnormalities that raise operative risk, intestinal bypass surgery may be considered for prophylaxis against long-term atherosclerotic and other complications. The procedure demonstrated joins a 12-inch segment of proximal jejunum to an 8-inch length of distal ileum and drains the bypassed gut into the sigmoid.

Since a tailored response can now be offered to the individual or couple choosing not to procreate, the physician and the hospital have an increased responsibility to counsel wisely as well as offer services. Moreover, physicians and hospital departments other than those most obviously concerned can play a role in this counseling. The author offers guidelines for helping an individual or couple make a mature decision.
