摘要:
Recognition of the clinical symptoms and signs that indicate the heart is no longer adequately fulfilling its prime mission as a pump depends less on the use of sophisticated techniques than on a systematic probing of the history, physical signs, and x-ray findings. A step-by-step approach is outlined through which a complete and harmonious picture may be obtained to serve as a basis for treatment.

摘要:
Simultaneous measurement of blood flow in a number of regional circulations has shown that infarction of the gastrointestinal tract may occur not only as the result of local occlusion but also as the remote consequence of a cardiac crisis. Conversely, cardiac output and coronary perfusion may be reduced by ischemic events in the splanchnic bed. The clinical implications of these findings are illustrated.

摘要:
In a number of infections (for example, tuberculosis) acquired resistance depends on a cell-mediated type of immunity rather than the classic antigen-antibody reaction. The mediating cell is a short-lived small lymphocyte, produced specifically in response to antigenic stimulus. In turn it mobilizes phagocytes at the infective site and activates them to become much more effective antibacterial agents.

摘要:
In contrast with the fragmentary data obtainable by auscultation, instantaneous, continuous recording of the fetal heart rate through a direct physical connection permits identification both of baseline patterns and deviations—especially certain types of deceleration—associated with fetal distress. The method is now sufficiently reliable to warrant its use at least in all high-risk deliveries.

摘要:
Should the acknowledged higher risk of a second primary breast cancer in women who survive their primary unilateral lesion mandate prophylactic removal of the contralateral breast? Or should this approach be reserved for those in extremely high-risk groups, with reliance on rigorous and meticulous follow-up for the others? Dr. Lewison details the evidence on both sides of these difficult questions.
