Alterations in the symptom pattern of previously recognized heart disease or the sudden appearance for the first time of disabling cardiac symptoms define a preinfarction state that mandates prompt intervention. In many patients the treatment of choice is myocardial revascularization by saphenous vein bypass. Criteria for selection, steps in the work-up, and experience in this group of patients are detailed.

Biologic substances occurring in such minute quantities that they eluded previous methods can now be assayed in vitro by a variety of nuclear techniques. Dr. Murphy discusses the quantitative and biologic assays made possible by the use of proteins to“capture”the target substances, or“ligands.”Dr. Wagner reviews the uses of radioactive carbon in the evaluation of metabolic events and anomalies in vitro rather than in vivo, and also discusses other methods, employing radioactivity measurements, that appear to offer significant potential for clinical application.

The theory that specific brain amines are depleted in depression and excessive in mania is called into question by studies using objective methods of rating patient behavior following administration of tryptophan and dopa, precursors of serotonin and dopamine. Serotonin appears not to be involved in either depression or mania. Excess dopamine is not a factor in depression but may be associated with mania in some individuals.

In about 30 inherited syndromes involving many organ systems, the heart, too, is affected; given the limited phenotypic expression of cardiac malformation, a lesion such as coarctation of the aorta may have a genetic basis in some patients. If so, the recurrence risk can be more or less defined for counseling purposes. Perhaps more often, however, an interplay of genetic predisposition and environmental trigger is responsible.

Since sickle cell disease mimics many other disorders, it may be misdiagnosed unless screening is performed routinely; in addition, its presence tends to complicate other diseases and require alterations in the way these are managed. Emphasis is placed on the avoidance of unnecessary surgery, the proper choice of anesthesia when it is necessary, and a variety of other measures that can help ward off crises.
