Demonstration that the aspirin-like compounds block prostaglandin synthesis has not only provided the long-sought explanation of how these drugs work but is throwing considerable light on the physiologic functions of the prostaglandins. It has also suggested possible new uses for the drugs (perhaps, for example, to prevent unwanted abortions) and new contraindications (e.g., where wound healing might be delayed).

When symptoms of cerebral and extracerebral ischemia arise from compromise of blood flow proximal to the carotid arteries, the hazards of thoracotomy or median sternotomy entailed in endarterectomy can be avoided by a carotid-to-subclavian side-to-side anastomosis, which employs a much less traumatic supraclavicular incision. The procedure is demonstrated in a patient with severe subclavian stenosis.

The first of two articles is primarily concerned with the use of injected iron isotopes to study the kinetics of erythroid production and of chromium-labeled red cells to evaluate red cell survival, sequestration patterns, and occult blood loss. By these means important data can be obtained concerning anemias whose fundamental etiology is not immediately apparent, often greatly altering the therapeutic approach.

A system enabling oviducal fluid to be collected continuously in animals has helped demonstrate that the metabolic events occurring in the tube play a crucial role in the reproductive process. With more knowledge of the environmental conditions necessary for fertilization, moreover, an in vitro system has been developed in which human ova have undergone cleavage following exposure to spermatozoa.

Understanding of the hemodynamic patterns associated with left ventricular dysfunction has been aided by more accurate techniques for monitoring cardiac function soon after infarction. These include a simpler method of obtaining pulmonary artery wedge pressures and noninvasive ways of evaluating left heart size and pulsation abnormalities. They may assist in applying bolder therapies early in the patient's course.
