More detailed knowledge of the sequence of changes in myocardium undergoing infarction, together with new biochemical methods of quantitating infarct size, has improved the prospect of pharmacologic and metabolic intervention to limit the extent of cell necrosis. Even modest gains in maintaining myocardial viability may be sufficient to permit subsequent definitive therapy by surgical or other means.

The antigenic specificity of a molecule found in tumor tissue of the G I tract has been established through a variety of immunologic studies; whether it can eventually provide the basis for a diagnostic test is not yet demonstrated. Since the substance is originally found in fetal cells, but only between the end of the second and sixth months of gestation, its reappearance in cancer may be associated with a derepression of genetic information.

For definitive treatment of cholelithiasis, an effective procedure for detecting the presence of stones in the common duct is essential; operative cholangiography provides it. Its routine use in conjunction with cholecystectomy, which has been the policy at the authors' institution for two decades, can contribute significantly to reducing the morbidity associated with repeat operations on the biliary tract.

Biochemical methods are differentiating these inherited anomalies in terms of the particular factor missing or defective in the cells of the affected individual. This approach has already shown that cells from individuals with almost any one of the syndromes (or from normals) will correct the abnormal mucopolysaccharide metabolism of cells from individuals with one of the other conditions. The implications for therapy are far-reaching indeed.

The full diagnostic potential of nuclear data cannot be realized without computer assistance, the authors say, describing a system that analyzes the images obtained from single scans in many different ways—quantifying the distribution of radioactivity on a regional basis, evaluating objectively the changes in function over time. Surprisingly, perhaps, this expanded capability can now be achieved at relatively little cost.
