摘要:

The mechanisms whereby the urate crystal initiates the acute attack of gout have been sought for more than a century; critical experiments are described through which it has been established that after the crystal's ingestion by the cell, hydrogen bonding occurs between crystal and lysosomal membrane. The result is membrane rupture, release of lysosomal enzymes, cell autolysis, and discharge of enzymes into joint fluid.

A rarity in the U.S. today, tetanus should not occur at all when a near-perfect method of protection is available in toxoid. Yet while many individuals are overimmunized, others—especially women over 20 and men over 50—have antibody titers below the protective threshold. Dr. Edsall calls for greater stress on ascertaining and improving the immune status of such high-risk individuals, less on the automatic“wound booster.”

Simultaneous imaging of RES marrow, spleen, and liver with the newer radiocolloids that have a short physical halflife is making it feasible to evaluate most of the elements of the reticuloendothelial system within the limits of a permissible radiation dose. Such“total”scans can be accomplished within an hour and provide important supplementary data in patients with a wide variety of hematologic disorders.

Syndromes associated with chromosomal anomalies can be identified unequivocally through karyotyping; with point mutations, the task of correct classification is more difficult. Concluding the series on medical genetics, Dr. McKusick points out that as subtle methods have been developed to differentiate phenotypically similar but genetically heterogeneous conditions, the number of discrete genetic entities has risen into the hundreds.
